Super Flexible Hospice Nurse - Field-Based - Full Time with Amazing Benefits! Brooklyn Coverage with a Low Caseload!

In this field-based position, you will work closely with our Interdisciplinary Team to provide direct nursing care, coordinate services, and support patients and their families throughout their end of life journey.

What You'll Do:

Patient Care & Coordination

Provide Direct Care: Deliver hands-on nursing care to hospice patients in their homes.
Plan & Implement Care: Collaborate with the Interdisciplinary Team to develop, evaluate, and revise each patient’s plan of care based on their changing needs.
Manage Services: Arrange for additional agency services as needed to ensure all patient and family needs are met.
Communicate Effectively: Keep physicians and other hospice staff informed of any changes in a patient’s condition and obtain necessary medical orders.
Supervise Staff: Prepare written instructions for and supervise Home Health Aides.
Team Collaboration: Actively participate in Interdisciplinary Team meetings, in-service trainings, and community education programs.

Documentation & Outreach

Clinical Documentation: Document all pertinent information in the clinical record, including evidence of coordinated care.
Intake Coordination: Assist in coordinating hospice intake as needed.
Patient Education: Explain hospice eligibility criteria and reimbursement options to patients and families.

Qualifications & Requirements

Must speak Chinese
Current NYS Registered Nurse License.
Experience: 1 year of RN expirience working with adults and/or geriatrics
Education: Graduate of an accredited school of nursing; a Baccalaureate Degree in Nursing is preferred.
Additional Requirements: Must have a valid driver’s license and a reliable car.

Schedule & Compensation

Schedule: Monday–Friday, with one weekend per month. Typical shifts are 9 AM–5 PM or 8 AM–4 PM.
Salary Range Based on Exp: $109K to $130K+
