Full Job Description

Position Summary

The Program Manager is responsible for the day-to-day oversight, coordination, quality, and regulatory compliance of services provided through Kentucky’s Acquired Brain Injury (ABI), Supports for Community Living (SCL), and Home and Community Based (HCB) waiver programs.

The Program Manager ensures that individuals receive safe, person-centered services consistent with their approved plans of care, person-centered service plans, agency policies, Medicaid requirements, and applicable Kentucky regulations. The Program Manager provides direct supervision and support to program staff and serves as a primary operational contact for participants, guardians, case managers/service coordinators, staff, and other members of the participant’s support team.

Essential Duties & Responsibilities

Program Oversight

Oversee daily operations of ABI, SCL, and HCB waiver services assigned to the Program Manager.
Ensure services are delivered according to each participant’s approved plan of care/person-centered service plan.
Monitor participant schedules, staffing coverage, service delivery, attendance, and program activities.
Ensure adequate staffing is maintained and address call-offs, absences, coverage issues, and other operational concerns.
Conduct routine program/site observations to monitor quality, safety, cleanliness, staffing, and participant care.
Assist with the implementation of new waiver programs and services as the agency expands.

Participant Care & Service Coordination

Become familiar with each participant’s goals, needs, preferences, restrictions, risks, behavioral supports, and service requirements.
Ensure staff understand and properly implement participants’ plans and objectives.
Monitor participant progress toward established goals and identify concerns requiring follow-up.
Communicate with participants, guardians, case managers/service coordinators, and other members of the interdisciplinary team as appropriate.
Participate in care conferences, team meetings, annual meetings, and other service-planning meetings as assigned.
Address participant and guardian concerns promptly and notify the Executive Director of significant issues.

Staff Supervision

Provide day-to-day supervision, direction, coaching, and support to Direct Support Professionals (DSPs), program staff, and other assigned employees.
Assist with interviewing, orientation, onboarding, and training of new employees.
Ensure employees understand participant-specific plans, agency policies, documentation requirements, emergency procedures, and job responsibilities before providing services independently.
Monitor staff attendance, punctuality, performance, professionalism, and compliance with agency requirements.
Provide coaching and corrective counseling and recommend disciplinary action when appropriate.
Report serious performance or conduct concerns to the Executive Director.
Assist with staff meetings and ongoing training.

Documentation & Compliance

Review service documentation for accuracy, completeness, timeliness, and consistency with authorized services.
Monitor daily notes, progress notes, goal/objective documentation, attendance records, medication-related documentation when applicable, incident reports, and other required records.
Ensure documentation supports the services actually provided and corresponds with participants’ plans and authorized services.
Identify missing, late, incomplete, or inconsistent documentation and ensure timely correction.
Maintain organized participant and program records in accordance with agency policy and applicable confidentiality requirements.
Assist in maintaining compliance with Kentucky Medicaid requirements and applicable Cabinet for Health and Family Services requirements for ABI, SCL, and HCB services.

Quality Assurance

Conduct routine reviews of participant records and program documentation.
Assist with internal audits and preparation for Medicaid, licensing, certification, or other regulatory reviews.
Develop and implement corrective actions when deficiencies are identified.
Monitor implementation of Plans of Correction and ensure corrective measures remain in place.
Track recurring documentation, staffing, safety, or service-delivery concerns and report trends to the Executive Director.
Participate in the agency’s quality improvement activities.

Minimum Qualifications

High school diploma or GED required; associate’s or bachelor’s degree in human services, social work, psychology, healthcare administration, business administration, or a related field preferred.
Previous supervisory or management experience in developmental disability, acquired brain injury, behavioral health, home- and community-based services, healthcare, or a related setting preferred.
Experience with Kentucky Medicaid waiver services strongly preferred.
Knowledge of ABI, SCL, and HCB programs preferred.
Strong organizational, documentation, communication, and leadership skills.
Ability to supervise employees and appropriately address performance and compliance concerns.
Ability to maintain confidentiality and handle sensitive participant information.
Ability to use electronic documentation, scheduling, and other agency systems.
Must successfully complete all required background checks, screenings, training, and other qualifications applicable to the position and services supervised.
Must maintain a valid driver’s license and acceptable driving record when driving is an essential function of the position.
