Overview

We are seeking a dedicated and highly skilled Registered Nurse/MDS Coordinator to join our healthcare team. This vital role combines clinical nursing expertise with comprehensive management of the Minimum Data Set (MDS) process, ensuring compliance with federal and state regulations while delivering exceptional patient care. The ideal candidate will possess a strong background in nursing, proficiency in electronic health record (EHR) systems, and a thorough understanding of Medicare and Medicaid requirements. This position offers an opportunity to influence quality improvement initiatives and optimize resident outcomes through meticulous documentation and case management.

Duties

Lead the completion and accuracy of the MDS assessments, ensuring compliance with CMS (Centers for Medicare and Medicaid Services) guidelines and long-term care regulations.
Collaborate with interdisciplinary teams to develop individualized care plans that meet clinical standards and regulatory requirements.
Review medical documentation for completeness, accuracy, and adherence to clinical documentation standards, including ICD-10/ICD coding, CPT coding, and DRG (Diagnosis-Related Group) classifications.
Conduct utilization management and review processes to support appropriate resource allocation, discharge planning, and case management.
Maintain proficiency in EMR/EHR systems such as Cerner, Epic, or Athenahealth to ensure seamless documentation workflows.
Provide education and training to staff on health regulation policies, HIPAA compliance, and documentation best practices.
Monitor changes in healthcare policies related to Medicare, Medicaid, HEDIS measures, and long-term care regulations to ensure ongoing compliance.
Facilitate audits and reviews of medical records for quality assurance purposes aligned with NCQA standards.
Support clinical documentation improvement initiatives aimed at optimizing reimbursement while maintaining high-quality patient care standards.

Experience

Registered Nurse (RN) licensure with current state certification required; prior experience in long-term care or skilled nursing facilities preferred.
Demonstrated expertise in MDS coordination, with a strong understanding of CMS regulations governing long-term care facilities.
Extensive knowledge of ICD-10/ICD coding systems, CPT coding procedures, and DRG classifications used in medical billing and reimbursement processes.
Proven experience with electronic health records (EHR) systems such as Cerner, Epic, or Athenahealth; familiarity with EMR/EHR management is essential.
Critical care experience or ICU background is advantageous; inpatient experience enhances clinical assessment skills necessary for accurate documentation review.
Familiarity with utilization review processes, discharge planning procedures, and case management principles within hospital or outpatient settings.
Strong understanding of healthcare policies related to HIPAA, Medicare regulations, HEDIS measures, and state healthcare laws governing medical records management.
Excellent organizational skills with the ability to multitask effectively in fast-paced environments requiring attention to detail and compliance adherence.
