Summary:
Clinically screen patients for nutrition risks and takes appropriate action. Consults at contract facilities as a representative of Faith Health.

Responsibilities:
Essential Job Duties and Responsibilities:
1. Demonstrates ability to appropriately modify approach and procedures to meet needs of age/diversity of population served.
- Neonate (birth – 28 days)
- Infant (29 days – less than 1 year)
- Pediatric/Child (1 year – 12 years)
- Adolescent (12 years – 18 years)
- Adult (18 years – 65 years)
- Geriatric (over 65 years)
- Incorporates cultural considerations in the provisions of care.
- Knowledge of growth and developmental stages.
- Considers life changes/effects on health beliefs and behaviors.
- Provides necessary safety measures.
- Provides information and involves family/caregiver in decision making.
2. Performs all responsibilities/duties required by the Nutrition Services Department as defined in the scope of practice, to assure that the unique nature of the patient is addressed. This includes but is not limited to the age of the patient served.
- Interacts professionally and treats patients and their families with respect and dignity.
- Demonstrates an ability to be flexible, organized and function under stressful situation.
- Maintains a good working relationship both within the department and with other departments.
- Assists in orienting and in-servicing new employees to the department.
- Conducts in-services and educational sessions as requested to hospital staff and the community.
3. Demonstrates knowledge and appropriate interventions in meeting the needs of the population served, as defined in the age-specific standards of care giving:
- Knowledge of growth and developmental stages pertinent to population served.
- Considers life changes/effects on health beliefs and behaviors.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Uses population specific knowledge to appropriately modify approach and procedures to meeting need of age groups served.
- Provides information and involves family/caregiver in decision making as appropriate.
4. Demonstrates the ability to accurately assess patients’ needs using organizational standards of practice guidelines.
- Assesses patients’ nutritional status according to established standards. Uses organization criteria for nutrition screening to target moderate and high-risk patients.
- Includes religious, cultural, and ethnic factors as well as patients’ wishes when completing nutrition assessments and care plans.
- Completes and updates nutrition care plan for all patients, if applicable, based on the nutrition risk level assigned to the patient following review of the medical record and following department policy for nutrition assessment and documentation.
- Evaluates the results of nutritional intervention on the patient’s nutrition status to determine the need to revise the nutritional care plan.
- Applies current research information to the delivery of optimal nutrition care.
- Documentation meets current standards and policies.
5. Makes appropriate recommendations for enteral tube feedings and parenteral formulas and makes recommendations regarding changes based on the nutritional needs of the patient.
- Identifies appropriate enteral products and supplements to meet patient needs based on organizational formulary.
- Accurately calculates protein, calorie and fluid requirements to meet age-specific needs of patients.
- Accurately calculates nutrient content of tube feeding and parenteral nutritional formulations.
6. Is knowledgeable of therapeutic diets and demonstrates the ability to use the organization’s diet manual as a resource.
- Demonstrates the ability to plan and correct menus for all diet modifications.
- Appropriately identifies food-drug interactions and makes necessary therapeutic changes in the nutrition care regimen to prevent interactions and provides patient education.
- Assist in planning and reviewing therapeutic, special and regular diet plans and menus to ensure they are in compliance with the physician’s order.
7. Identifies the need for education and documents the education process in the patient’s medical record and care plan.
- Appropriately incorporates knowledge of therapeutic diets when conducting patient education.
- Identifies religious, cultural, motivational and other barriers that may impact a patient’s response to learning.
Identifies and utilizes resources available in the organization, such as interpreters, specialized materials or educational devices, to enhance the learning process.

Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

Job Requirements: The requirements listed below must be representative of the knowledge, skills, minimum education, training, licensure, experience, and/or ability required.
EDUCATION:
Bachelor’s Degree required.
EDUCATION FIELD OF STUDY:
Nutrition/Dietetics or related field required.
CERTIFICATES, LICENSES, REGISTRATIONS:
Registered Dietitian with the Commission on Dietetic Registration or be RD eligible (registration exam must be passed prior to start date) required.
Current Licensed Medical Nutrition Therapist (LMNT) license from the state of Nebraska required.
EXPERIENCE:
Previous healthcare experience preferred.
SKILLS:
Language Skills – Ability to read, write, speak, and understand the English language required.
Other Certifications/Requirements:
Current, valid driver’s license issued in the state of legal residence required.
