We are seeking a dedicated professional who is passionate about children and coming alongside others to make a difference in their lives.

Be part of our team. CHKD is seeking a professional and caring registered nurse or licensed clinical social worker to join a growing care management program across the clinically integrated network, Fortify Children’s Health. This role is remote and maintains a typical schedule of full-time, days.

The licensed care manager is uniquely positioned to establish trusting relationships for the well-being of children and families. The goal of this position is to improve patient outcomes, enhance patient experiences, and reduce healthcare costs by coordinating and managing care in collaboration with their medical home.

An ideal candidate will possess:

· Current Virginia state license as a Registered Nurse or Registered Nurse holding a valid Compact State license, or as a License Clinical Social Worker (LSCW).

· Strong understanding of Care Coordination best practices.

· Proficient computer skills

· 2+ years of relevant professional care management experience

· Strong organizational skills and the ability to work independently

· Excellent communication skills and the ability to interact with diverse individuals

· Preferential consideration given for experience working with children, families, community partners or in a behavioral health setting.

· Prior work with Medicaid population a plus.

· Experience conducting medication reconciliation with patients and identifying discrepancies.

· Experience communicating with medical providers regarding patient needs.

· Comfort working in a fast pace and dynamic “startup” environment.

· Bi-lingual (English/Spanish) preferred.
