Full Job Description

Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate patient care. Your expertise will help optimize patient outcomes, improve care quality, and enhance patient satisfaction. This position offers an exciting opportunity for nurses with a passion for case management and comprehensive patient assessment to make a meaningful difference in patients’ lives.

Responsibilities

On-call 24/7 for client emergencies and medication issues as they arise.
Ensure all staff in the medical department are following Danielle’s Law and Stephen Komninos’s Law.
Oversee the daily operations of the nursing case management department.
Active participant in all Pre-admission/placement meetings.
Review all pre-admission medical information/history to ensure the agency can meet the medical needs of the individual.
Attend all medical appointments/meetings for new and more serious medical diagnoses/treatments (e.g., surgeries, cancer, cardiac, pulmonary/respiratory, renal failure, etc.).
Conduct all required medical related trainings for managers, staff, etc. when there is a new client admission and new manager hire and new employees hire trainings (bimonthly nursing training for licensing)
Responsible for disability forms
Responsible for all Prior Auths
Responsible for all chart notes for medical equipment
Must attend all ISP, IDP, and ITP meetings
Liaison for families, guardians, case managers, support coordinators, hospitals, sub acutes, etc.
Responsible for all hospital discharges and ensure all medical documents are received, reviewed, and communicated to all parties.
Ensure hospital discharge procedures are followed as per the agency’s policy.
Follow up on all client hospitalizations daily/regularly & provide updates to all departments, families/guardians.
Assess hospitalized clients if needed to ensure they are at baseline, stable and ready for re admission.
Responsible for all client education regarding various medical related issues, conditions, diagnoses, refusal of medical orders/treatment, etc.
Obtain prescriptions & discontinued medication Rxs for managers as needed.
Prepare physician notes for missed medications, medication discrepancies, etc. and ensure the physician reviews & signs the note.
Ensure compliance with regulatory agencies such as the State of NJ, DDD and state/local department of health.
Work with the pharmacy, insurance companies Report any Rx issues regarding insurance/payment issues, drug interactions, etc.
Review medical related incident reports daily.
Work with physicians to ensure all clients are evaluated as medically indicated.
Address medical concerns as presented by all agency staff.
Assist with nursing clinical duties if needed.
Conduct weekly team meetings.
Attend and participate in weekly administrative meetings.
Evaluate and address disciplinary actions for the medical department if needed with RN Supervisor.
32. Evaluate, complete and review annual appraisals for medical staff with RN Supervisor.
Seek out additional expertise and advice when needed to assure clients needs are met and proper practice is followed.
Maintain open communication with medical, residential, rehab & day program departments and regularly communication with families, guardians, case managers, etc.
Attend & participate in weekly management meetings.
Must cooperate with licensee and department staff with any inspections or investigations.

Pay: Up to $150,000.00 per year
