Care 4 U Community Health Center is seeking a compassionate, versatile, and experienced Dual-Certified Family Nurse Practitioner (FNP) and Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) to join our multidisciplinary healthcare team.
This unique hybrid position is particularly well-suited for a practitioner who wants to meaningfully use both their FNP and PMHNP training. The position offers the opportunity to provide both primary and psychiatric care while serving a diverse patient population across the lifespan.
The provider will divide their week between two Care 4 U clinical locations, with the clinical focus varying by site:
Tuesday through Thursday, the provider will practice at our clinic located within the Lotus House Village in Overtown, where they will utilize both their FNP and PMHNP certifications to provide integrated primary and behavioral healthcare to women and children residing at Lotus House, as well as individuals and families throughout the surrounding Overtown community.
Monday and Friday, the provider will practice at our main Liberty City clinic, where their role will be primarily focused on psychiatric and behavioral healthcare. The provider will work alongside our existing primary care practitioners and behavioral health team to conduct psychiatric evaluations, provide medication management, and help Care 4 U continue to develop and expand its behavioral health services.
Across both locations, the provider may care for individuals experiencing behavioral health conditions, chronic medical conditions, homelessness, trauma, substance use disorders, HIV, increased vulnerability to HIV and other sexually transmitted infections, and other barriers to accessing consistent healthcare.
The ideal candidate is an autonomous, compassionate clinician who enjoys caring for the whole person and working with individuals and families across the lifespan. They should be comfortable caring for both pediatric and adult patients, understand the impact that trauma and social circumstances can have on physical and mental health, and be committed to providing trauma-informed, patient-centered, culturally responsive, and nonjudgmental care to individuals from diverse backgrounds.
Core Responsibilities
Behavioral Health & Psychiatry
- Conduct comprehensive psychiatric evaluations and mental health assessments for patients across the lifespan.
- Diagnose and treat behavioral and mental health conditions, including depression, anxiety, PTSD, bipolar disorder, ADHD, schizophrenia, and other psychiatric conditions within the provider's scope of practice.
- Develop, implement, and manage individualized psychiatric treatment plans.
- Prescribe and monitor psychotropic medications in accordance with applicable federal and Florida requirements.
- Monitor treatment effectiveness, medication adherence, potential side effects, and patient progress, adjusting treatment plans as clinically appropriate.
- Provide ongoing psychiatric medication management and follow-up care.
- Provide care using trauma-informed principles, recognizing that some patients may have experienced homelessness, domestic violence, abuse, chronic trauma, or other significant life stressors.
- Collaborate with primary care providers, behavioral health counselors, case managers, and community partners to ensure coordinated treatment and appropriate referrals.
- Assist Care 4 U in continuing to build, develop, and expand its integrated behavioral health and psychiatric services.
Family, Women's & Pediatric Primary Care
- Provide comprehensive family primary care to patients across the lifespan, including children, adolescents, women, and adults.
- Conduct routine physical examinations, health assessments, preventive screenings, and age-appropriate wellness visits.
- Diagnose and treat acute illnesses and minor injuries and provide appropriate follow-up care.
- Provide preventive and routine women's healthcare within the provider's scope of practice.
- Provide pediatric and adolescent primary care, including routine health maintenance, developmental and preventive screenings, and treatment of common acute and chronic conditions.
- Diagnose, treat, and manage chronic health conditions such as hypertension, diabetes, asthma, hepatitis C, and other common medical conditions.
- Provide health education and counseling to promote wellness, disease prevention, medication adherence, and healthy lifestyle choices.
- Make appropriate referrals for specialty care and coordinate follow-up to promote continuity of care.
HIV Prevention, Treatment & Substance Use Care
- Provide or support comprehensive medical care for patients living with HIV, including monitoring treatment, laboratory results, and adherence to antiretroviral therapy.
- Educate, screen, and counsel patients regarding HIV prevention, including PrEP (Pre-Exposure Prophylaxis) and PEP (Post-Exposure Prophylaxis), when clinically appropriate.
- Screen for and manage sexually transmitted infections and other health concerns associated with increased HIV risk.
- Screen patients for substance use disorders and provide appropriate treatment, counseling, referrals, and care coordination.
- Incorporate evidence-based, trauma-informed, and harm-reduction approaches when caring for individuals affected by substance use.
- Provide Medication-Assisted Treatment (MAT) when clinically appropriate and within the provider's training, licensure, and scope of practice.
- Collaborate with behavioral health, case management, outreach, and other members of the multidisciplinary team to address medical, behavioral, and social factors affecting treatment outcomes.
Integrated Care & Coordination
- Serve as a clinical bridge between primary care, behavioral health, case management, and community-based services.
- Collaborate with Care 4 U's multidisciplinary clinical team and community partners to address patients' medical, behavioral, and social needs.
- Develop patient-centered treatment plans that consider both physical and behavioral health needs.
- Coordinate referrals and follow-up care with specialists, hospitals, behavioral health providers, social service organizations, and other community resources as appropriate.
- Participate in multidisciplinary case conferences and care coordination activities as needed.
- Maintain accurate, timely, and compliant documentation within the Electronic Health Record (EHR).
- Adhere to organizational policies, clinical protocols, quality standards, and applicable federal and state requirements.
- Participate in quality improvement, clinical education, and other organizational initiatives as appropriate.
Required Qualifications
- Master's or Doctoral degree in Nursing from an accredited program.
- Active national board certifications as both a Family Nurse Practitioner (FNP) and Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC).
- Active, clear, and unencumbered Florida APRN license.
- Current DEA registration with authority to prescribe controlled substances, as applicable.
- Eligible for enrollment and credentialing with Medicare, Medicaid, and commercial health plans.
- Current BLS certification or ability to obtain prior to employment.
- Ability to successfully complete all required criminal background screenings and other pre-employment requirements.
- Strong clinical judgment and ability to practice independently within the APRN's authorized scope of practice.
- Strong interpersonal, communication, and interdisciplinary collaboration skills.
Preferred Qualifications
- Experience providing psychiatric and behavioral healthcare, including psychiatric evaluation and medication management.
- Experience providing primary care to women, children, adolescents, and families.
- Experience working with underserved or medically vulnerable populations.
- Experience working with individuals or families who have experienced homelessness, trauma, domestic violence, or other significant social determinants of health.
- Experience in community health, integrated primary/behavioral healthcare, or a similar setting.
- Experience providing HIV care and/or HIV prevention services, including PrEP and PEP; HIV-specific training or certification is a plus.
- Experience diagnosing and treating substance use disorders and/or providing Medication-Assisted Treatment.
- Strong knowledge of trauma-informed care and harm-reduction principles.
- Bilingual in English/Spanish or English/Haitian Creole is highly desirable.
Pay: $115,000.00 - $160,000.00 per year
