Full Job Description

Northridge Kidney Center LLC, a growing dialysis provider in the San Fernando Valley, is seeking an experienced and compassionate Registered Dietitian (RD) to join our team. Our organization is built on strong relationships, accessible leadership, and a shared commitment to our patients and our people. Our Registered Dietitian (RD)will work closely with leadership and the clinical team, with the opportunity to make a meaningful impact on patient care, staff development, and the overall success of the facility.

The Registered Dietitian (RD)plays a key role in ensuring safe, effective, and compassionate patient care. This position works closely with the Medical Director, nursing team, clinical professionals, administrative staff, patients, and families to promote positive clinical outcomes and improve the overall dialysis experience.

We believe exceptional dialysis care goes beyond the treatment itself. Patient education, communication, compassion, and customer service are essential to helping patients feel informed, respected, and supported throughout their care. Our Registered Dietitian (RD) will lead by example, developing a team that consistently delivers individualized care while maintaining the highest standards of clinical quality, patient safety, and professionalism.

Key Responsibilities

· Foster a positive, welcoming patient environment that reflects the organization's values and commitment to compassion, clinical excellence, and exceptional customer service.

· Perform comprehensive nutritional assessments using laboratory results, treatment information, weight trends, medical history, and other clinical indicators to identify nutritional risks and develop individualized nutrition care plans.

· Provide ongoing ESRD and dialysis-specific nutrition counseling and education, including fluid management, sodium, potassium, phosphorus, protein, diabetes, weight management, and other nutrition-related conditions.

· Assess patients' understanding of kidney disease and nutritional needs, identify barriers to dietary and treatment adherence, and develop practical strategies tailored to each patient's medical condition, lifestyle, preferences, culture, and learning needs.

· Educate patients and families on healthy eating, lifestyle modifications, disease management, and available resources to support long-term health and quality of life.

· Monitor patient progress and clinical outcomes and modify nutrition interventions as needed based on changes in laboratory values, weight, medical condition, and treatment needs.

· Collaborate with the Medical Director, nurses, social workers, physicians, patients, families, and other interdisciplinary team members to develop and implement comprehensive, patient-centered care plans.

· Participate in monthly patient rounds, interdisciplinary care planning, QAPI activities, and other clinical quality initiatives.

· Maintain accurate and timely EMR documentation, including assessments, care plans, interventions, education, follow-up, and other required clinical information.

· Maintain knowledge of nutrition science, physiology, metabolism, renal disease, diabetes, weight management, and dietary interventions relevant to the dialysis population.

· Support dietary operations as applicable, including menu planning, dietary modifications, and compliance with applicable regulatory and facility standards.

· Maintain compliance with applicable State and Federal regulations, CMS Conditions for Coverage, facility policies, professional standards, and infection prevention requirements.

· Participate in staff meetings, in-services, continuing education, audits, and quality improvement activities.

· Maintain appropriate infection control practices and use PPE as required to protect patients and staff.

· Demonstrate strong communication, organization, clinical judgment, and time-management skills while managing multiple patient needs in a fast-paced dialysis environment.

· Perform other duties as assigned to support the clinical and operational needs of the facility.

Qualifications

· Registered Dietitian (RD) credential required.

· Bachelor's degree in Nutrition, Dietetics, or related field.

· Renal or dialysis experience strongly preferred.

· Knowledge of ESRD, hemodialysis, renal nutrition, fluid management, and common dialysis-related comorbidities.

· Strong assessment, critical-thinking, and clinical problem-solving skills.

· Excellent communication and patient education skills.

· Ability to work independently while collaborating effectively with an interdisciplinary team.

· Commitment to patient-centered care and exceptional customer service.
