Housing Navigator

REPORTS TO: Housing Program Manger

WORK HOURS: Average 40 hours per week; schedule varies; possibly includes one

evening per week. Additional hours as needed to fulfill job duties.

FLSA STATUS: Exempt

DATE: 7/20/2026

SUMMARY

To support family and single households in preparing for, securing, and retaining permanent housing, by assessing barriers to permanent housing, developing goals, objectives and plans that address the barriers, which includes obtaining and retaining full-time permanent employment and other income; to provide and access supportive services including individual mental health services and monitor progress made toward achieving the goals, objectives and ISPs.

RESPONSIBILITIES

Assesses barriers and needs of incoming families and develops individual service plans (ISP), including goals, objectives, and action steps, to address each of the barriers/needs
Establishes program expectations with incoming families and monitor compliance. When expectations are not being met, recommends and implements appropriate actions to address shortcomings.
Regularly meets with program participants to review ISP progress and modify plans as needed to ensure positive outcomes.
Identifies supportive services required by program participants in order to achieve long and short-term goals and provides and/or accesses applicable supportive services and resources. Accompany participants to appointments as needed.
Supports program participants in seeking out and obtaining safe, affordable and decent permanent housing as well as in realizing employment outcomes that will support family in permanent housing.
Enhances a Trauma Informed Service Environment. Treats all clients with respect, dignity and hospitality. Maintains a high level of customer service.
Initiates appropriate action and crisis intervention with individuals, including as needed, prompt utilization of community resources.
Leads and/or supports the development and implementation of internal programming established to support families in overcoming common barriers.
Prepares and maintains notes and files that are adequate to demonstrate efforts of program participant and their case manager/CPST. Ensure compliance with productivity standards and documentation for agency, funding and accreditation requirements by maintaining and reporting accurate and timely documentation and statistical data.
Where outcomes are below expected levels, works closely with supervisor in addressing ways to bring outcomes to expected levels in near-term.
Develop and maintain collaborative relationships with landlords, property managers, and housing providers throughout Cuyahoga County where Eden housing vouchers are accepted to expand housing opportunities for program participants.
Assist residents with creating and navigating their CMHA applicant portal, completing required documentation, and monitoring application status to support timely access to permanent housing opportunities.
Assist residents with identifying and securing appropriate housing, conducting searches, completing rental applications, and coordinating appointments to view available units.
Support residents throughout the housing placement process by ensuring all move-in requirements are completed, including coordinating utility activation and arranging for furniture delivery prior to occupancy.
Collaborate with case managers, landlords, and community partners to facilitate successful housing placements and address barriers that may delay move-in.
Attend mediations as it relates to the resident’s housing as needed to advocate for residents and support housing stability.
Maintain accurate documentation of housing search activities, landlord contracts, client progress, and housing placement outcomes in accordance with agency policies and funding requirements.
Educate residents on the terms and conditions of their lease, ensuring they understand their rights, responsibilities, and expectations as tenants to promote successful housing retention.
Complete a comprehensive warm handoff to the assigned Case Manager following housing placement by sharing relevant information, coordinating the transition of services, and ensuring continuity of care and ongoing support.

SKILLS AND KNOWLEDGE

Strong communications skills.

Experience with crisis intervention and/or mediation.
Good knowledge of community resources.
Ability to work effectively as part of a team.
Strong computer skills.
Understanding and supportive of a trauma-informed system of care.
Experience working directly with people from diverse, racial, ethnic, and socioeconomic backgrounds.
Excellent written and verbal communication skills and the ability to communicate specifically with an audience from diverse racial, ethnic, and socioeconomic backgrounds.
Have knowledge and skills to understand landlord and tenant rights

ADDITIONAL REQUIREMENTS

Valid Ohio driver’s license with good driving record and proof of insurance.
Hours of work may exceed normal business hours; ability to work non-traditional work hours including weekends and holidays. Will be available for one evening per week and one Saturday per month for face-to-face contact with program participants, when needed.
Familiarity with working in a Trauma Informed Environment.

The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, and the ability to adjust focus.

The noise level in the work environment is usually moderate. While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions.

This position requires the employee to work with a diverse population, therefore, there may be exposure to potentially infectious material. Other exposures could include cleaning supplies, chemicals involved in pest control, paint and other materials used in building maintenance.
