Full Job Description

Gladeview Rehabilitation and Healthcare Center is seeking a Part -Time Physical therapist to work with our dynamic short-term rehab and long term care population! Gladeview is situated in scenic Old Saybrook, CT overlooking Ragged Rock Marina and Long Island Sound. One of the few remaining privately owned and operated skilled nursing facilities in CT.

Duties as a PT at Gladeview include, but are not limited to the following:

~ Assessment/evaluation of patients over a myriad of diagnoses and development of a plan of care to focus on the ultimate goals of the patient’s stay.

~ Progress physical therapy treatment and supervise/advise assistants according to state regulations with the same regard.

~ Work closely with an interdisciplinary team including occupational therapy, speech therapy, respiratory therapy, social services, and nursing to provide quality and comprehensive treatments driven by function and patient focused goals.

~ Report information regarding patients (progress, changes, tolerance) to the interdisciplinary team, including care plan meeting attendance, as appropriate.

~ Provide patient, caregiver, and staff training as directed and appropriate.

~ Participate in discharge planning including home evaluations as deemed necessary.

~ Complete electronic documentation in a thorough and timely manner.

~ Represent and participate with department and building marketing as instructed.

Qualifications for the position:

~ Licensure in good standing with the state of Connecticut. The use of temporary licensure can be discussed

~ Good communication skills along with a team driven attitude

~ Competency with computers and completion of electronic medical records

~ Organization, flexibility, and responsibility to patients, coworkers, and administration
