Mobile Primary Care Registered Nurse (RN) – Full-Time Hybrid Field & Remote Role

We're looking for an experienced, compassionate, and motivated Registered Nurse (RN) to join our growing mobile primary care team.
In this unique hybrid role, you'll work side-by-side with an experienced provider, bringing high-quality healthcare directly to patients in their homes, assisted living communities, and other residential settings throughout Salt Lake, Davis, and Utah Counties.
Approximately 40% of your time will be spent providing direct patient care in the field, while 60% will be spent working remotely from home completing clinical documentation, care coordination, chart reviews, patient communication, and other clinical administrative responsibilities.

What You'll Do
- Travel with the provider to patient visits in homes, assisted living communities, and other residential settings.
- Perform comprehensive nursing assessments and identify changes in patient condition.
- Assist the provider during examinations, procedures, and treatment planning.
- Document patient encounters accurately and efficiently in the electronic medical record (EMR).
- Complete chart reviews, care coordination, follow-up communication, and other clinical documentation from your home office.
- Communicate effectively with patients, families, caregivers, pharmacies, and healthcare facilities.
- Help coordinate care plans and ensure patients receive timely follow-up and support.
- Maintain high standards of clinical quality, professionalism, and patient confidentiality.
- Provide compassionate, patient-centered care while representing the practice in the community.

Required Qualifications
- Current, active Registered Nurse (RN) license in good standing in the state of Utah.
- Valid driver's license, reliable transportation, and ability to travel throughout Salt Lake, Davis, and Utah Counties.
- Comfortable driving in all seasons, including winter weather conditions.
- Strong clinical assessment skills with the ability to recognize changes in patient condition and communicate concerns appropriately.
- Excellent written and verbal communication skills.
- Strong organizational skills with the ability to manage time, prioritize tasks, and work independently.
- Strong computer literacy with the ability to efficiently navigate EMR systems, accurately complete clinical documentation, type patient notes, manage electronic communication, and complete remote administrative tasks independently.
- Comfortable working in a hybrid role that includes both in-home/community patient care and remote clinical documentation from a home office.
- Ability to maintain patient privacy and confidentiality while working remotely.
- Ability to maintain a professional home workspace with reliable internet access for completing clinical documentation and communication.
- Professional, dependable, and committed to providing compassionate, high-quality care.
Preferred Qualifications
- 2+ years of nursing experience preferred.
- Experience in primary care, geriatrics, home health, hospice, skilled nursing, or community-based healthcare.
- Experience working with medically complex patients or individuals with disabilities.
- Experience assisting providers during outpatient visits or procedures.
- Familiarity with care coordination, chronic disease management, and communication with healthcare facilities.
- Experience using mobile healthcare technology, telehealth tools, or electronic medical record systems.

Compensation & Benefits
- Competitive pay: $36–$44 per hour, based on nursing experience, clinical skills, and qualifications.
