Job Description
The Connecting H.O.P.E. ( Healing Opportunity Perseverance Empowerment) Program works with non-custodial parents who have active Florida child support cases and are unemployed or underemployed at the time of enrollment. Connecting H.O.P.E. is an employment-focused program designed to assist participants in overcoming barriers that prevent them from securing employment and increasing income.
POSITION SUMMARY
The Case Manager is a critical member of the Connecting H.O.P.E. program team, responsible for providing comprehensive case management services to parents who are navigating employment barriers and child support obligations. Our service area spans across 8-counties in Central Florida. The Case Manager serves as a primary point of contact when a parent has shared critical needs and obstacles that impact their ability to obtain and maintain gainful employment.
We are seeking a Case Manager that has experience with barrier identification, is able to research, find community resources and connect parents with resources to meet critical needs identified during barrier assessment. The selected Case Manager will also be responsible to provide follow-up support services and tracking to meet specific deliverables and benchmarks. Case Manager is expected to work with parents as needed for the duration of parent enrollment and participation in the H.O.P.E program.
The purpose of the Connecting H.O.P.E ( Healing, Opportunity, Perseverance & Empowerment) is to strengthen families and communities by equipping parents with the employment services, community resources and parent-centered support and empower them in gaining stability, self-sufficiency and an overall better quality of life for themselves and their loved ones.
Program covers the following counties: Orange, Osceola, Lake, Seminole, Sumter, Volusia, Polk, and Putnam counties.
KEY RESPONSIBILITIES
Case Management & Barrier Reduction
- Conduct comprehensive assessments upon parent enrollment to determine critical needs and barriers to gainful employment
- Identify and address barriers to employment including transportation, legal issues, housing instability, child support obligations, and mental health needs
- Connect participants to specific occupational skills training, trade and career pathways certifications and opportunities for upward mobility
- Track and document parent progress and retention milestones including 30/60/90/180-day employment retention milestones
- Coordinate referrals to internal and external community partners including legal services, co-parenting education, financial literacy, and mental health resources
- Facilitate warm handoffs to partner organizations including and other resources as needed.
- Maintain accurate, up-to-date participant files in Salesforce CRM and Google Drive per program file structure guidelines
- Ensure all participant records meet the Connecting H.O.P.E. Standard for Completeness
- Provide ongoing, strengths-based case management to an assigned caseload throughout the program cycle
Employment Support & Job Placement
- Collaborate with the Employment Coordinator and Employment Specialist to connect participants to employer opportunities aligned with their career pathway goals
- Facilitate referrals to vocational training, trade certification programs, and credentialing opportunities through program partners
- Track and document parent progress and retention milestones including 30/60/90/180-day employment retention milestones
Documentation & Compliance
- Maintain complete, accurate, and timely documentation of all participant interactions, services, referrals, and outcomes in Salesforce CRM and program files
- Submit required data and documentation for monthly funder reports and program deliverable tracking
- Participate in file audits, quality assurance reviews, and program improvement processes as directed by the Program Manager
- Adhere to all confidentiality requirements, HIPAA guidelines, and Cornerstone Connections organizational policies
Community Engagement & Outreach
- Participate in outreach events, job fairs, and community engagement activities across the 8-county service area as assigned
- Represent Connecting H.O.P.E. professionally in community settings, partner meetings, and employer engagements
- Support the Program Manager in implementing place-based outreach strategies within assigned service area
Team Collaboration
- Participate in weekly team meetings, individual 1-on-1 check-ins with the Program Manager, and required training and professional development activities
- Collaborate with Employment Coordinator, Employment Specialist, and Intake & Training Specialist to provide seamless, coordinated participant services
- Contribute to a team culture of accountability, excellence, dignity, and hope-centered practice.
Qualifications
- Required
- Bachelor’s degree in Social Work, Human Services, Psychology, Counseling, Public Administration, or a closely related field
- Background in nonprofit/social services work
Preferred
- Master’s degree in Social Work, Counseling, Human Services, or related field
- Experience with Salesforce CRM or similar nonprofit case management platforms
- Bilingual proficiency (English/Spanish/Creole) preferred
- Experience in workforce development, child-support obligation, parent programs, or fatherhood/family stability programs
- Familiarity with Florida child support system, family court processes, and community resources across Central Florida
- Experience with funder compliance, deliverables and outcome reporting
- Minimum 2 years of experience in case management, workforce development, social services, or a related field
- Demonstrated experience working with populations facing employment barriers, reentry challenges, child support obligations, or similar circumstances
- Proficiency in Microsoft Office Suite, Google Workspace, and experience with CRM or case management database systems
- Valid Florida driver’s license and reliable transportation
- Strong written and verbal communication skills with the ability to produce professional documentation and reports
