Full Job Description

Job Posting – Now Hiring: Housing Case Manager
Location: Leeway

Department: Case Management

As of August 2026, Leeway, Inc. is seeking one qualified individual to join our team as a Housing Case Manager. These positions play a vital role in supporting individuals with behavioral health needs in achieving housing stability through coordinated, person-centered case management and housing support.

Key Responsibilities:

Assess Housing Needs:

Screen referrals and evaluate eligibility for housing programs.
Conduct comprehensive housing barrier assessments and needs evaluations.

Housing Coordination:

Identify and secure suitable housing options for clients.
Coordinate lease agreements and maintain relationships with landlords and property managers.

Case Management & Client Support:

Develop and implement individualized housing support plans.
Conduct regular home visits and provide ongoing case management services.
Refer clients to external services such as benefits and entitlements, mental health, housing, and addiction support programs.

Compliance & Documentation:

Maintain accurate and timely case records in electronic health record systems.
Prepare required reports for program compliance and participate in audits and quality assurance reviews.

Required Qualifications:

A minimum of an associate degree in a behavioral health related field or with two (2) years of college training in a behavioral health or rehabilitation therapy field; or
An individual with one (2) or more years of experience working with individuals with mental illness involving participation in an interdisciplinary team process and the development, review, and implementation of an individual’s plan of services (completion of a mental health trainee program issued by a college or university may be substituted for six (6) months of the General Experience)
Valid driver’s license and access to reliable transportation to conduct home visits and attend external meetings.
