Job Title: Part-Time MDS Nurse (RN/LVN)
Location: Glenwood Care Center
Position Type: Part-Time
License: RN preferred; LVN accepted

RN PAY : $50-$55

LVN: $36-$38

Job Summary:
Glenwood Care Center is seeking a detail-oriented and knowledgeable MDS Nurse to join our team on a part-time basis. The MDS Nurse plays a critical role in ensuring accurate resident assessments, care planning, and compliance with state and federal regulations within a skilled nursing facility.

Key Responsibilities:

Complete and coordinate the Minimum Data Set (MDS) assessments in accordance with CMS guidelines
Ensure timely and accurate submission of MDS to support appropriate reimbursement
Collaborate with interdisciplinary team (nursing, therapy, dietary, social services) to develop and update individualized care plans
Monitor and track resident outcomes, ensuring quality measures are met
Maintain compliance with all federal, state, and facility regulations
Participate in care plan meetings and communicate resident needs effectively
Review documentation to ensure accuracy and support coding on MDS assessments
Assist with audits and survey readiness related to MDS and care planning

What an MDS Nurse Does in a Skilled Nursing Facility:
An MDS Nurse is responsible for assessing residents’ clinical conditions and translating that information into standardized data used for care planning, quality reporting, and Medicare/insurance reimbursement. This role is essential in ensuring residents receive appropriate care while the facility remains compliant and financially accurate.

Qualifications:

Active California RN license (preferred) or LVN license (required minimum)
Previous MDS experience in a skilled nursing facility strongly preferred
Strong understanding of CMS guidelines and reimbursement systems (PDPM)
Excellent attention to detail and organizational skills
Ability to work collaboratively with interdisciplinary teams
Strong communication and documentation skills
