Care Manager Unit Lead: Middlesex & Somerset County

About Us

Families usually come to us when things feel overwhelming, a health decline, a crisis, or too many decisions at once. Peace Care Experts exists to bring steadiness, clarity, and reassurance in those moments.

With more than twenty years of experience, we provide private-pay, relationship-based care management for older adults, adults with complex care needs, and their families across nine counties of Northern and Central New Jersey.

The Role

You'll carry your own care management caseload while leading a growing unit of care managers across Middlesex and Somerset County.

You'll be supported and mentored alongside your fellow unit leads who collaborate closely and rely on each other. The team you're leading has built real trust with each other, and you'll have the room to shape what leadership looks like for them as we scale.

This is a role for someone who leads by example: steady under pressure, clear in communication, and genuinely invested in the people on their team.

About You

You follow through on what you say you'll do, and people notice. You've built trust with families and providers over time, and you're ready to build that same trust with a team.

Qualifications

Care Management

● Active NJ clinical license (RN, Social Work, OT, or PT)

● Care management, geriatrics, or home health experience required

● Calm, compassionate presence and strong clinical judgment

● Excellent communication with families, providers, and team members

Unit Leadership

● Prior supervisory, mentoring, or team lead experience preferred

● CCM or CMC certification a plus

● Comfortable holding both a caseload and a leadership role at once

Key Responsibilities

Care Management

● Conduct comprehensive assessments and develop individualized care plans

● Build trusted relationships with clients and families

● Coordinate care across providers and services

● Attend key medical appointments and support medication oversight

Unit Leadership

● Review team communications to ensure quality and consistency

● Hold weekly one-on-one check-ins with care managers on your unit

● Manage vacation coverage and scheduling for your team

● Participate in weekly leadership meetings

● Share periodic on-call rotation for after-hours client needs

Role Details

● Full-time, Monday through Friday, with occasional flexibility for on-call coverage

● Hybrid: Home office, client visits across Middlesex and Somerset County, and team meetings

● All client and team leadership work paid: Visits, calls, emails, documentation, travel, appointments, and lead duties

● Overtime and holiday pay: Rare after-hours or weekend work billed at overtime rates

● Company-provided: Laptop and cell phone, technology support, and ongoing mentoring from senior leadership

Compensation & Benefits

● Starting at $65.00/hour, based on experience, plus an additional generous stipend for unit lead responsibilities

● Mileage reimbursement

● PTO and sick pay

● 401(k) with employer match

● Health Reimbursement Arrangement (HRA)

● $250 annual continuing education credit
