Full Job Description

Job Purpose

Reports to the Director of Housing and Resident Services. The Licensed Social Worker provides behavioral health support, case consultation, and therapeutic services for current and alumnae moms across one home, providing trauma-informed services that strengthen mental health, resilience, and overall well-being.

Responsibilities:

Behavioral Health Support

In conjunction with the Case Manager and external counselors, develops customized treatment plans.
Provide moms with the resources they need to reach personal counseling goals.
Monitor client progress and modify their case plans as needed.
Complete continuing education courses to maintain certifications & stay up-to-date on latest practices.
Utilizes behavioral interventions, coping skills strategies to support reaching personal goals and overall self sufficiency.
Document all clinically significant interactions including assessments, interventions and addressed issues within 72 hours to facilitate continuity of care.
Refers and coordinates continuity of care with outside counseling resources as necessary.
Seeks consultation with staff team members as necessary.
Provides crisis intervention and coordinates emergency services as needed and/or available.
Adheres to confidentiality law and procedures at all times.
Obtain signed release of information authorization form from client as required by CSWMFT
Maintains appropriate professional boundaries and adheres to ethical standards in all interactions.
Track and input mental health data to client records in the reporting system on a regular basis.
Responsible for maintaining data and reporting outcomes on a monthly basis to the Director of Housing and Resident Services
Collaborate with the Grants Committee to support the continuous improvement of program data collection, outcomes measurement, and impact reporting.
Facilitates Seeking Safety curriculum for current moms on an individual and group basis.
Develop and adapt Seeking Safety session plans to meet the specific needs of participants.
Monitor & evaluate the effectiveness of the Seeking Safety curriculum & update as necessary.
Participation, collaboration and occasional facilitation of house meetings, enrichment activities and productivity hour for residents on Programming day:

Intake Management

Supervise the JVC Intake Coordinator to ensure consistent intake phone coverage and timely response to all inquiries Monday–Friday, 9:00 a.m.–5:00 p.m.
Oversee the intake process by ensuring in-person interviews are scheduled promptly and conducted within established timelines.
Conduct in-person intake interviews and coordinate all required follow-up, including references, documentation, and additional information.
Collaborate with the appropriate Case Manager, Social Worker, and program staff to determine applicant placement and ensure timely admissions decisions.
Communicate intake decisions to applicants and coordinate move-in logistics with the receiving home's team.
Facilitate a smooth handoff to the assigned Case Manager and Social Worker following acceptance.
Maintain accurate, confidential intake records and oversee the organization's intake tracking system and related data.
Coordinate room readiness, move-in logistics, and a welcoming move-in experience for new residents.
Maintain proactive communication regarding applicant status, scheduling, and intake coverage to ensure continuity throughout the move-in process.

Collaborative Efforts

Serves in the on-call rotation for in-home emergencies.
In conjunction with Social Worker, offer guidance and support to interns, addressing any issues or concerns that arise.
Participate in weekly and monthly meetings with appropriate teams for easy and transparent information sharing.
Coordination of intern support in your home with the Intern Program Manager.
Collaborate with the leadership team to facilitate the necessary protocol prior to asking a mom to leave. If issues arise and/or final decision is needed, consult the Director of Resident and Housing
Provides transportation for moms and their children on as needed basis with company vehicles.
Educate clients on stress and trauma coping mechanisms.
Educate staff on stress, trauma coping mechanisms, ACEs, trauma informed care etc.
Be available for emergency/crisis/major issues that arise at the house. Provide onsite support to the staff during crisis or emergency situations. Be intentionally aware of conflicts, difficulties or stressors. Coordinate with a mental health provider to deescalate the situation. Follow up with the Executive Director.
Mediates and leads important conversation/safe space conversations with moms, staff, volunteers etc as needed.
Assumes any additional projects or tasks as assigned by the Director of Housing and Resident Services.

Qualifications:

Bachelor's degree in social work, counseling, or other related therapy
Licensed LSW or LISW in the state of Ohio
Master’s degree, preferred
At least 2 years of social work experience
Valid Driver's License
Knowledge of resources in the Cleveland community
Strong computer skills.
Strong interpersonal skills and engagement skills.
Ability to work effectively as part of a team.
Demonstrates experience and competency in working in a culturally diverse setting, and an understanding of the intersectional nature of homelessness, poverty, abuse, disability, substance abuse and mental illness.
Excellent organizational, supervisory, and communication skills, with proficiency in engaging diverse audiences.
Experience working with vulnerable populations, addiction and poverty.
Experience with crisis intervention and/or mediation.
Alignment with Zelie’s Home core values and sanctity of human life.
Must be able to lift up to 25 pounds occasionally (e.g., moving donations, baby supplies, or household items).
Ability to climb and descend stairs multiple times per day, as the home includes multiple floors.
Comfortable working in both office and residential settings, with frequent movement between rooms and interaction with families
