Full Job Description

Responsibilities:

The Peer Navigator will work closely with the Lead Navigator and Project Director to organize agency and community engagement activities, provide HIV and substance misuse prevention and education services, and support individuals at highest risk for HIV and substance use disorders. This position operates within a Substance Abuse and Mental Health Services Administration (SAMHSA) funded and monitored program, requiring adherence to federal grant guidelines, reporting requirements, and the implementation of evidence-based prevention practices.

This role is primarily field-based and requires regular travel throughout the community to engage with external partner agencies, schools, and community organizations to deliver outreach activities and prevention curriculum. The Peer Navigator must be a highly motivated individual who demonstrates initiative and is able to work independently with minimal supervision, while also contributing effectively within a collaborative, multidisciplinary team in a fast-paced service environment.

The Peer Navigator must be able to independently manage field-based responsibilities, be proactive in building and maintaining community partnerships, and take initiative in identifying outreach and engagement opportunities within the service area. This role requires a self-directed professional who coordinates community engagement activities, demonstrates initiative in program promotion and community outreach, and is resourceful in navigating community systems and partnerships to connect participants with appropriate services and supports. Strong written and verbal communication skills are essential for representing Eggleston Youth Centers and maintaining productive relationships with community stakeholders and partner organizations.

Duties:

1. Coordinate participant intakes and assessments.

2. Coordinate treatment plans and discharge planning.

3. Provide case management and care coordination to program participants.

4. Plan, organize, and conduct community and agency outreach activities, including engagement with external partners and stakeholders.

5. Deliver HIV and substance misuse prevention programming using Evidence-Based Practices (EBPs) in community and field settings in alignment with SAMHSA grant requirements.

6. Promote program services through outreach events, community presentations, and partnerships with local agencies.

7. Represent Eggleston Youth Centers at community meetings, outreach events, and collaborative initiatives.

8. Ensure coordination with internal agency services and external community-based organizations to support participant recovery and continuity of care.

9. Facilitate drug and alcohol education groups within the outpatient program setting, utilizing harm reduction principles and evidence-based prevention curricula to support departmental prevention efforts and promote healthy decision-making among participants.

10. Coordinate HIV and viral hepatitis testing services with appropriate providers and partners.

11. Provide UA testing as required by program protocols.

12. Review charts and documentation to ensure compliance with Title 22 regulations and SAMHSA grant requirements.

13. Monitor participant progress and update treatment plans as needed.

14. Participate in peer reviews and case consultations.

15. Participate in weekly supervision and team meetings.

16. Complete required annual training hours and professional development.

17. Participate in program evaluation, performance improvement, data collection, and quality assurance initiatives related to grant performance metrics.

18. Adhere to all agency safety protocols and procedures.

Position Requirements:

1. Bachelor’s Degree in Social Services or related discipline, or Equivalent combination of education and work experience.

2. Registered or Certified with (CCAPP, CADTP, CAADEE)

3. Valid California driver’s license and insurable driving record.

4. Live Scan fingerprinting clearance for FBI, DOJ and CAI.

5. Health screening clearance.

6. Demonstrates ability to work as part of a multi-disciplinary team.

7. Excellent organizational, written and oral communication skills.

8. Must be willing to travel/drive throughout Los Angeles County SPA (3) and (6) to meet the designated position duties as indicated.

Physical Skills and Abilities:

1. Must have the visual, physical and technical ability to independently operate a variety of office equipment including the use of a computer.

2. Must be capable of bending, stooping, filing, lifting of at least 25 lbs. and able to interrupt physical altercations.

3. Must have the ability to verbally communicate well with others

1. Interfaces with all levels of management, staff, clients, residents and families including partner agencies.
