West Janisch Health Care Center is seeking an experienced MDS Nurse (RN or LVN) to join our clinical leadership team. This position is ideal for an experienced MDS professional who understands the clinical, regulatory, and reimbursement aspects of skilled nursing and takes pride in delivering accurate assessments that support exceptional resident care.

What You'll Do

As our MDS Nurse, you'll play a critical role in ensuring accurate resident assessments, regulatory compliance, quality outcomes, and appropriate reimbursement by:

Completing and coordinating accurate, timely MDS 3.0 assessments for Medicare skilled and long-term care residents.
Managing Assessment Reference Dates (ARDs), assessment scheduling, and timely CMS submissions.
Completing Care Area Assessments (CAAs) and collaborating with the interdisciplinary team to develop and update individualized care plans.
Leading and facilitating resident care plan meetings with residents, families, physicians, therapy, dietary, social services, and nursing.
Reviewing clinical documentation to ensure accuracy, regulatory compliance, and appropriate reimbursement.
Monitoring Quality Measures, CASPER reports, and other clinical outcome reports while identifying opportunities for improvement.
Collaborating with physicians and interdisciplinary team members to ensure documentation supports resident needs and services provided.
Supporting survey readiness, quality assurance initiatives, regulatory compliance, and facility audits.
Remaining current on CMS regulations, the RAI Manual, Medicare and Medicaid reimbursement guidelines, and industry best practices.
Providing clinical support and education to nursing staff as needed.

QualificationsRequired

Current, unencumbered Texas RN or LVN license
Current CPR certification
Previous MDS experience in a Skilled Nursing or Long-Term Care setting
Experience completing Medicare (Skilled) and Long-Term Care MDS assessments
Strong knowledge of:
MDS 3.0
RAI Manual
Care Area Assessments (CAAs)
Care Planning Process
Medicare & Medicaid reimbursement
Quality Measures and regulatory compliance
Strong organizational, analytical, and communication skills
Ability to work independently while collaborating effectively with an interdisciplinary team

Preferred

RAC-CT Certification
Experience with PointClickCare (PCC) or similar EMR systems
Wound care documentation and coding experience
Case Management, Rehab, or Clinical Reimbursement experience
