Full Job Description

MDS Coordinator (RN)

Seven Acres Jewish Senior Care Services is seeking a detail-oriented and knowledgeable MDS Coordinator (RN) to join our team. This is an excellent opportunity for a licensed nurse with MDS experience who is ready to support accurate resident assessments, care planning, regulatory compliance, and reimbursement processes. In this role, you will work closely with the interdisciplinary team to ensure assessments are completed timely, documentation is accurate, and each resident’s care plan reflects their individual needs.

Key Responsibilities

Coordinate and complete MDS assessments and care plans in a timely and accurate manner
Ensure compliance with CMS, state, and facility regulations
Collaborate with the interdisciplinary team to develop, review, and update resident care plans
Monitor clinical documentation to ensure accuracy, compliance, and reimbursement support
Maintain knowledge of MDS guidelines, PDPM requirements, and regulatory updates
Participate in quality assurance and performance improvement activities
Communicate effectively with nursing, therapy, administration, and other care team members
Support accurate reimbursement through thorough documentation review and assessment completion

Qualifications

Current and active state RN license
Previous MDS experience in a skilled nursing facility preferred
Strong understanding of CMS regulations, reimbursement systems, and PDPM
Strong attention to detail and commitment to accuracy

Preferred Qualifications

Experience in a skilled nursing or long-term care setting
Familiarity with EMR systems and MDS software
Knowledge of care planning, quality measures, and survey readiness
Prior experience supporting audits, documentation review, or reimbursement processes
