Overview

Join our dedicated healthcare team as a Home Visiting Nurse, where your expertise will directly impact patients' lives by providing compassionate, comprehensive nursing care in the comfort of their homes. This vital role combines clinical excellence with personalized patient service, ensuring individuals receive the highest quality care tailored to their unique needs. As a Home Visiting Nurse, you will collaborate with healthcare professionals to deliver critical assessments, education, and support, promoting health and well-being in community settings. Your proactive approach and clinical skills will help bridge the gap between hospital and home, fostering recovery and independence for diverse patient populations.

Responsibilities

Conduct thorough patient assessments including vital signs, physiological evaluations, and medical history reviews to determine care needs.
Deliver personalized nursing care in patients’ homes, including wound management, medication administration, and disease management.
Collaborate effectively with physicians, case managers, social workers, and other healthcare providers to develop and implement individualized care plans.
Manage patient records accurately using Electronic Health Records (EHR) systems while ensuring compliance with HIPAA regulations.
Educate patients and families on health conditions, medication use, discharge planning, and post-acute care strategies to promote self-care.
Monitor patient progress through ongoing evaluation and adjust care plans accordingly to optimize health outcomes.
Coordinate services such as hospice care, post-acute recovery, or chronic disease management while managing caseloads efficiently.

Qualifications

Valid nursing license with current registration in the applicable state.
Proven experience in critical care environments such as ICU, emergency rooms, or trauma centers; Level I or Level II trauma center experience preferred.
Strong background in hospital medicine, medical-surgical units, or home health nursing with acute care experience.
Knowledge of infectious disease protocols, geriatrics, behavioral health, and post-acute care practices.
Proficiency in EMR/EHR systems and understanding of healthcare regulations including HIPAA and health policy standards.
Excellent patient service skills with the ability to perform comprehensive patient assessments and clinical case management.
Ability to communicate effectively with diverse patient populations and collaborate seamlessly with multidisciplinary teams.
Familiarity with diagnostic evaluation techniques, managing patients as a nurse case manager, and discharge planning processes.
