Job Summary
Join our dynamic healthcare team as a Registered Nurse in a Long-Term Acute Care (LTAC) setting, where your expertise will directly impact patient recovery and quality of life. In this vital role, you will provide specialized nursing care to patients with complex medical needs, ensuring safe, effective, and compassionate treatment. Your skills in critical care, advanced assessment, and interdisciplinary collaboration will drive positive outcomes for individuals transitioning from intensive hospital settings to long-term recovery environments. This position offers an exciting opportunity to apply your diverse clinical experience in a fast-paced, supportive environment dedicated to excellence in patient-centered care.

Duties

Deliver comprehensive nursing assessments and develop individualized care plans tailored to each patient’s needs.
Administer medications, including IV infusions and injections, following strict aseptic techniques and medication protocols.
Monitor vital signs, perform physical examinations, and evaluate physiological responses to treatments.
Manage complex medical procedures such as ventilator support, catheterization, wound suturing, and tube feeding.
Collaborate with interdisciplinary teams to coordinate care plans, discharge planning, and post-acute services.
Document all patient interactions accurately using EMR (Electronic Medical Record) systems like Epic or Cerner while maintaining HIPAA compliance.
Provide education and support to patients and their families on health management, medication administration, and recovery processes.

Requirements

Valid RN license with current state licensure or eligibility to practice.
Critical care experience in ICU or trauma center settings is highly preferred.
Proficiency with EMR/EHR systems such as Epic or Cerner; experience with ICD coding (ICD-9/ICD-10) is a plus.
Demonstrated expertise in managing complex conditions including trauma medicine, infectious disease care, pain management, and hospice & palliative medicine.
Experience working with individuals with developmental disabilities or memory care needs is advantageous.
Strong knowledge of physiology, anatomy, and diagnostic evaluation techniques.
Ability to perform advanced procedures such as lab specimen collection/processing, phlebotomy, sterile processing, and aseptic technique adherence.
Excellent patient assessment skills combined with the ability to triage effectively in urgent situations.
Familiarity with long-term care environments including assisted living facilities and nursing homes; home & community care experience is beneficial.
Certifications in Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and other relevant specialties are required.
