Full Job Description

The Outpatient Case Manager's goal is to provide services to clients with serious and persistent mental illness that will help them achieve their personal recovery goals,and increase their quality of life. The Case Manager will be expected to work with a diverse population and must be sensitive to the culture, religion, language, age, gender, and sexual orientation of the population served.

ESSENTIAL DUTIES

1) Develops and implements individualized treatment plans in collaboration with the client, focusing on the skills needed to successfully live and work as independently in the community as the client is able.

2) Educates clients regarding the role of medication compliance in maintaining stability.

3) Develops healthy and effective working relationships with clients,

their families, agency staff, psychiatrists, and other community supports. Maintains good professional boundaries with clients and their families.

4) Works with clients in the community, assisting with developing skills to achieve or sustain independence. Participates in outreach to client’s home as needed.

5) Plans for and addresses consumers' health care needs. Coordinates care with the agency RN and with clients’ primary care providers. Accompanies clients to medical health care appointments as appropriate.

6.) Addresses substance use issues with clients within the framework of Harm Reduction. Uses Motivational Interviewing as appropriate.

6) Effectively intervenes in client crises and assists with and facilitates hospitalization if needed. Writes affidavits for civil involuntary commitment of clients and coordinates with King County Designated Crisis Responders as needed.

7) Assists consumers in developing meaningful daily activities and positive social relationships. Also assists in helping consumer pursue occupational and educational goals as desired by consumer.

8) Conducts outreach to clients’ homes on a regular basis and as needed. Assists with maintaining their housing, and coordinates with the Supported Housing Manager.

9.) Ensures that consumers meet their appointments, within and outside the agency; may accompany clients to appointments if needed.

10) Carries a caseload of approximately 40 to 45 clients; maintains responsibility for high quality, professional records and other necessary documentation and completes it in a timely manner. Uses our Electronic Health record to document services, monitor and renew benefits and keeps clients’ records updated.

11.) The Case Manager is expected to engage in direct services with clients a minimum of 60 percent of their worktime, and document those services appropriately.

12.) Maintains quality documentation in accordance with agency, King County and WA state requirements.

13) Assists with or facilitates client intakes to services in accordance with their educational degree and Agency Affiliated Counselor status.

14.) Facilitates a minimum of one therapeutic or recreational group for clients per week.

15.) Coordinates with Supportive Housing staff to assist clients with maintaining and/or acquiring housing.

16.) Must be able to tolerate unruly, unpleasant, or bizarre client behavior.

17.) Must be able to work on-site, remote work is not available for this position.

18.) Performs additional duties as assigned.

RELATIONSHIP WITH COMMUNITY AND CO-WORKERS

1) Contributes to cooperative and effective working relationships with co-workers, other clinical staff, and supervisory staff within the agency.

2) Solicits and provides case consultation and coordination as needed.

3) Presents a positive professional image of the agency to referral sources, other service providers and community resources.

MINIMUM QUALIFICATIONS

BA or MA in Human Services plus two years treatment experience working with individuals with serious and persistent mental illness. Other social service experience may substitute. Knowledge of the King County system of public funded behavioral health services preferred. The position requires agency affiliated counselor registration, a valid driver's license, an automobile, first aide/CPR training, TB blood test, criminal background inquiry, and fingerprint ID. The employees driving record must be acceptable in order to drive the Agency’s vehicles and transport clients. The employee must show proof of personal automobile insurance. Transitional Resources reimburses agency related travel at the standard rate set by the federal government to help cover the cost of insurance, fuel, and wear on the automobile.

PHYSICAL REQUIREMENTS

The requirements described here are representative of those needed to fulfill the basic requirements of the position. Must be able to walk up and down stairs several times a day and lift up to 30 pounds if needed.

COVID-19 REQUIREMENTS

The agency follows CDC and Washington State Department of Health’s guidelines in its measures to prevent the spread of COVID-19. Must be able/willing to wear a mask if needed during an outbreak of Covid19 at the program. Must be willing to work with clients who may be positive for Covid19 and may be unwilling to wear a mask.

BENEFITS

Benefits include 22 paid leave days per year of service and 10 paid holidays, each pro-rated per fte; medical and dental insurance; long term disability insurance; voluntary participation in one the Agency sponsored 403(b) retirement plans and AFLAC and FSA cafeteria plans.

HOURS AND WAGES

Monday through Friday, days.

$42.86 per hour for BA Level and $48.83 per hour MA related level. Non-exempt position.
