Agape Care – Site Manager Job Description

Position Title: Site Manager

Reports To: CEO

Location: Agape Care Homes & Limitless Day Centers

Position Summary
The Site Manager is responsible for overseeing the daily operations of assigned homes and/or day centers, ensuring high-quality care, compliance with all Ohio Department of Developmental Disabilities standards, and delivery of Agape Care’s mission of Legendary Service. This role ensures staff accountability, consumer safety, and operational excellence across all assigned sites.

Key Responsibilities
1. Daily Operations Oversight

Oversee day-to-day operations of assigned homes and day centers
Ensure proper staffing ratios are maintained at all times
Monitor daily schedules, programming, and activities
Ensure a clean, safe, and structured environment for all consumers
Conduct routine site visits and walkthroughs

2. Staffing & Coverage Management

Serve as on-call contact for all call-offs, emergencies, and staffing needs
Coordinate coverage for call-offs and last-minute schedule changes
Assist with staff scheduling and shift adjustments
Ensure staff arrive on time and are fulfilling their responsibilities

3. Staff Supervision & Accountability

Provide direct supervision, coaching, and support to staff
Enforce Agape Care policies, procedures, and expectations
Address performance concerns promptly and professionally
Assist with training new hires and ongoing staff development

4. Compliance & Documentation

Ensure compliance with DODD, Medicaid, and agency policies
Monitor and verify accurate documentation (daily notes, attendance, incident reports)
Ensure EVV (Sandata) compliance for all services
Review and follow Individual Service Plans (ISPs)
Ensure all paperwork is submitted timely (timesheets, attendance, mileage logs)

5. Consumer Care & Safety

Ensure the health, safety, and well-being of all consumers
Respond to incidents and ensure proper reporting procedures are followed
Support staff in managing behaviors and implementing positive interventions
Communicate with families, guardians, and SSAs as needed

6. Communication & Coordination

Maintain clear and consistent communication with leadership
Participate in leadership and team meetings
Communicate updates, concerns, and changes to staff
Ensure proper handoff communication between shifts

7. Program & Quality Improvement

Ensure engaging, person-centered programming is being delivered
Support community outings and activities
Identify areas for improvement and implement solutions
Maintain high standards of service aligned with Agape Care’s brand

Additional Duties

Assist with tours for prospective families
Support onboarding and orientation of new consumers
Ensure transportation coordination when applicable
Step in to provide direct care support when needed

Qualifications

Experience working with individuals with developmental disabilities
Leadership or supervisory experience preferred
Knowledge of DODD rules and compliance
Strong communication, problem-solving, and leadership skills
Ability to manage multiple sites and priorities
Reliable transportation and flexibility with schedule

Site Manager Compensation Package
1. Base Pay

$25/hour × 25 hours/week

$625/week
$32,500/year

2. On-Call Compensation

Weekday: $75/day
Weekend: $125/day

Estimated (5 weekdays/week):

$375/week
$19,500/year

3. Performance Bonus (after 6 months)

Annual Bonus: Up to $2,000 total per year

Based on:

Attendance & coverage (minimal call-off issues)
Compliance (EVV, documentation, audits)
Staff retention & leadership
Guardian/family satisfaction

Can be paid:

Annually

Split into quarterly payouts ($500 each) based on performance

4. Benefits Package
