Full Job Description

Hiring Immediately!!

Our Facility is seeking to hire a Full-time Assisted Living and Memory Care Wellness Director LPN or RN . This is a rewarding career opportunity for an individual that wants to make a profound positive difference in the lives of those he/she serves. They must possess compassion and a desire to work with Assisted Living, Alzheimer's/Dementia Residents as well as the families of those residents. The Wellness Director oversees the Resident Care Employees in ensuring quality resident care is provided with dignity and grace.

**We offer a great salary, competitive benefits package and a phone allowance. This is an exempt position.

Duties & Responsibilities:

Must be available for an on-call 24 hour rotation.
Ensure that all systems for providing and tracking resident care are completed in accordance with the residence policies and procedures.
Complete non-PCS care plan for all residents in the neighborhood.
E-MAR order management for residents.
Complete resident new admit/re-admission assessment.
Complete pharmacy consults and collaborate with pharmacy staff as needed.
Collaborate with Resident Care Consultant on clinical tasks.
Complete 5 — 10 med pass observations per month.
Quarterly audit 25% of all resident charts.
Responsible for completing all care profiles on a quarterly basis.
Handles incident report tracking, intervention and shared risk agreements.

Requirements:

Must have an unrestricted LPN or RN license.
Minimum of two years supervisory or management experience in the healthcare field.
Minimum of two years working with Assisted Living, Alzheimer's and Dementia residents.
Computer proficient with in-depth experience in Excel, Word & Outlook.
Maintain a good driving record, valid driver's license, reliable transportation and auto insurance.
Must be able to work flexible hours including evenings, weekends, special events and holidays.
Must be willing to communicate with the family of residents through phone calls, emails and text messages.
