Full Job Description

JOB TITLE: Wraparound Program Coordinator

Full-Time Exempt: $70,720-90,000 annual salary, depending on level of education and experience

PURPOSE OF THE POSITION:

The Wraparound Program Coordinator plays a vital role in streamlining the referral and care coordination processes for Wraparound families while supporting team organization, documentation, and communication. This position collaborates closely with the Wraparound Program Manager to ensure alignment, accountability, and timely, coordinated service delivery. The Program Coordinator proactively partners with the team to ensure that all aspects of the Plan of Care are tracked, updated, and implemented effectively while maintaining compliance with High Fidelity Wraparound (HFW) standards.

This role requires flexibility in scheduling to meet the needs of the families we serve, including evening and weekend availability when required. Participation in an on-call rotation is required.

Reports to Wraparound Program Manager.

MINIMUM QUALIFICATIONS:

Education:

Bachelor’s degree in Social Work, Psychology, or related field. Master’s degree preferred. Equivalent education/experience combination may be considered.

Experience:

Direct experience with child-serving systems (e.g., mental health, child welfare, probation, special education).
Experience facilitating team processes.
Prior experience with High Fidelity Wraparound preferred.

Other Requirements:

Valid unrestricted driver’s license, reliable transportation, and vehicle insurance.
Ability to work flexible hours, including evenings and weekends.
Must pass a Department of Justice/Fingerprint and Child Abuse Index clearance.

KEY RESPONSIBILITIES:

Program Coordination & Workflow Management

Manage Wraparound intakes, ensuring accurate documentation and timely engagement in alignment with HFW standards
Collect, track, and submit all required documents for program entry and service delivery
Serve as the central point of coordination for communication between families, providers, and system partners
Track and manage timelines, deadlines, and phases of the Wraparound process, ensuring all team members are informed of expectations and progress remains aligned with High Fidelity Wraparound standards
Coordinate service providers and support onboarding of contractors as needed
Schedule, prepare, and coordinate Family Team Meetings (FTMs)
Support program workflows including scheduling, reporting, and record maintenance
Track referrals, services, and assigned tasks to ensure follow-through, accountability, and timely completion
Identify delays or barriers and communicate proactively to maintain progress

Plan of Care Management

Track, update, and maintain Plans of Care, ensuring goals, tasks, and timelines are clearly documented and actively followed
Ensure alignment across team members by clearly communicating Plan of Care expectations
Monitor progress and update Plans of Care to reflect current family needs and functioning
Ensure services and interventions remain aligned with identified family goals and outcomes

Documentation & Compliance

Maintain complete and compliant case files within Fidelity EHR and required systems
Ensure all required documentation, releases, and fidelity tools (e.g., WFI, TOMS) are completed accurately and on time
Ensure documentation meets organizational, contractual, and legal requirements
Complete referrals, service documentation, and required forms to support service delivery

Financial Oversight & Family Service Fund Management

Manage and track family specialized service funds in alignment with the Plan of Care
Maintain accurate financial documentation and monitor expenditures
Coordinate billing processes and ensure timely and accurate submission of invoices
Ensure compliance with organizational policies, funding requirements, and audit standards
Monitor budget utilization and communicate discrepancies or concerns to leadership

Teamwork & Collaboration

Collaborate with multidisciplinary team members to ensure coordinated and consistent service delivery
Communicate case updates, progress, and barriers clearly and in a timely manner
Support team problem-solving and alignment around family needs and service planning
Perform additional duties as assigned to support program operations and organizational needs

Systems Collaboration: Ability to navigate and coordinate across multiple child-serving systems while maintaining family voice, reducing fragmentation, and promoting shared accountability.
Communication: Clear, empathetic, and professional communication with families, service providers, and team members.
Attention to Detail: High level of accuracy in meeting timeline, documentation, compliance, and tracking service outcomes. Ability to schedule, track referrals, and manage timelines to ensure smooth service delivery.
Trauma-Informed Practice: Understanding and applying trauma-informed care principles to support families effectively.
Problem-Solving: Critical thinking skills to assess needs, identify barriers, and implement creative solutions.
Financial Oversight: Ability to manage and track specialized service fund budgets, ensuring fiscal accountability, accurate documentation, and compliance with funding requirements.
