THE OPPORTUNITY
Vizx Corp is conducting a confidential search for an Executive Director to lead a community-rooted healthcare services organization in Los Angeles County. This is a full executive appointment — encompassing P&L ownership, CDSS Community Care Licensing regulatory authority, and strategic leadership of an organization serving a richly diverse population across one or more licensed program lines.
The Executive Director will be the senior-most on-site leader, reporting directly to the Vizx Corp CEO. Depending on the specific programs in scope, this role may oversee a Residential Care Facility for the Elderly (RCFE), an Adult Residential Facility (ARF), a licensed Home Health Agency, or a combination of these alongside In-Home Supportive Services (IHSS) coordination, Community-Based Adult Services (CBAS), and CalAIM Enhanced Care Management. You will be expected to navigate California's Title 22 regulatory environment with confidence and precision.
This role is for an executive who has operated within California's CDSS Community Care Licensing framework — not someone learning it. If you hold or have held an RCFE Administrator Certificate, California NHA license, or equivalent California credential, and have managed a licensed facility or multi-program organization in this state, we want to speak with you.
SCOPE OF LEADERSHIP
This is a full organizational leadership role. The Executive Director oversees:
1. Clinical & Program Operations
- RCFE, ARF, and/or licensed Home Health Agency program delivery and oversight
- CBAS, IHSS coordination, and/or CalAIM ECM as applicable to program scope
- MDT structure and supervision across all enrolled program lines
- Caregiver and direct care worker recruitment, training, evaluation, and support
- Clinical assessments, care plans, prior authorization, and documentation workflows Financial & Administrative Oversight
- Full P&L accountability across all licensed programs
- Medi-Cal revenue cycle: billing, claims, AR aging, prior authorization
- Budget management, vendor contracts, and cost controls
- Financial reporting to Vizx Corp leadership; Medi-Cal rate-setting compliance
2. Regulatory & Compliance Authority
- CDSS CCL Title 22 compliance for all licensed facility types (RCFE and/or ARF and/or Home Health)
- License maintenance, annual renewal, DPH and CCL inspection readiness
- DHCS Medi-Cal provider enrollment, billing compliance, and NPI/taxonomy maintenance
- CARF or ACHC accreditation ownership — monitoring, surveys, corrective action
- Incident reporting to CCL; HIPAA and data governance compliance Workforce & Organizational Leadership
- Executive leadership of licensed facility and program staff

KEY RESPONSIBILITIES
1. Clinical Operations & Service Delivery
- Lead delivery of services across all program lines with full accountability for member outcomes, caregiver quality, and program integrity
- Maintain the multidisciplinary team (MDT) structure required under applicable regulations — ensuring each function is appropriately staffed, supervised, and performing
- Oversee caregiver and direct care worker recruitment, onboarding, annual evaluation, and ongoing support to sustain program quality and member safety
- Ensure timely completion of all clinical documentation: assessments, prior authorizations, care plans, and discharge planning
- Monitor clinical quality indicators and drive continuous improvement across all care delivery programs
2. Regulatory Compliance & Risk Management
- Maintain full compliance with CDSS Community Care Licensing (Title 22 CCR, Division 6) for all applicable licensed facility types — which may include RCFE, ARF, and/or licensed Home Health Agency
- Own facility license maintenance, annual renewal, and inspection readiness — including DPH and CCL survey preparation and corrective action plan submissions
- Ensure DHCS Medi-Cal billing and provider enrollment compliance, including NPI, taxonomy codes, and prior authorization workflows
- Maintain CARF or ACHC accreditation as applicable — internal monitoring, policy compliance, mock surveys, and Standards conformance
- Oversee California Mandated Reporter obligations (Penal Code 11166); ensure all staff complete required certification training
- Monitor CDSS, DHCS, CDPH, and LA County DPH regulatory developments and implement policy changes proactively
- Maintain current DOJ/FBI LiveScan clearance and OIG/LEIE exclusion check compliance for all staff and contractors
3. Financial Leadership
- Own the organization's P&L — managing budget, cost structure, and financial performance to margin and growth targets set with Vizx Corp leadership
- Oversee revenue cycle performance: billing accuracy, claims submissions, AR aging, prior authorization compliance, and payer reconciliation
- Identify and execute opportunities for financial efficiency, service-line growth, and sustainable scaling
- Deliver regular, board-quality financial performance reporting to Vizx Corp leadership
4. Workforce Leadership & Organizational Culture
- Lead, develop, and retain a high-performing executive and clinical team — with clear expectations, ongoing feedback, and visible accountability
- Own talent strategy: workforce planning, succession readiness, and key-person risk mitigation in a competitive labor market
- Build and maintain a culture of mutual respect, clinical excellence, and organizational pride across a diverse workforce
- Serve as a stabilizing, accessible executive presence for staff, caregivers, and community partners throughout organizational transition
5. Stakeholder & Community Relations
- Build and maintain referral relationships with LA County hospitals, skilled nursing facilities, social workers, PCPs, and community-based organizations
- Serve as the primary executive representative with Medi-Cal managed care plans including L.A. Care, Health Net, Molina, and CalOptima
- Engage with DHCS, the LA County IHSS Public Authority, MSSP program offices, and CalAIM ECM lead entities as applicable to program scope
- Cultivate relationships with culturally and linguistically appropriate community organizations across the Los Angeles service area
- Communicate with members, families, and caregivers in their primary language or through qualified interpreter services
6. Organizational Transition & Integration
- Partner with Vizx Corp's Director of Administration & Operational Systems on system onboarding, policy harmonization, and operational alignment
- Build the executive team and operational infrastructure required for sustained performance at scale in the Los Angeles, CA market
- Develop and execute a 12-month strategic roadmap anchored in quality outcomes, regulatory compliance, and program growth
QUALIFICATIONS & LICENSURE
Licensure & Background Check Requirements — Los Angeles, CA
- An active California RCFE Administrator Certificate is required if this role includes oversight of a licensed Residential Care Facility for the Elderly. A California Home Health Agency Administrator credential or equivalent is expected if the scope includes a licensed Home Health Agency. Candidates overseeing an Adult Residential Facility (ARF) should hold or be eligible for relevant CDSS licensing credentials. Candidates with an NHA, LCSW, MFT, or RN license are strongly considered across all program types. All candidates must complete DOJ/FBI LiveScan fingerprinting and OIG/LEIE exclusion verification prior to hire.

- Mandated Reporter: California Mandated Reporter (Penal Code 11166) — certification required prior to direct member contact
Required Qualifications
- 7+ years of progressive healthcare leadership, with at least 3 years in an executive or senior director role in California
- Active or recent California RCFE Administrator Certificate (required if role includes RCFE oversight), NHA license, LCSW, or equivalent California credential
- Demonstrated P&L ownership in a California-licensed RCFE, ARF, home health agency, or multi-program community-based services organization
- Direct operational experience within CDSS Community Care Licensing (Title 22) regulatory framework for one or more facility types (RCFE and/or ARF and/or Home Health)
- Financial fluency in Medi-Cal billing, managed care payer contracts, and healthcare revenue cycle management
- Demonstrated ability to lead diverse, multilingual teams in a complex multicultural service environment
- Exceptional communication and relationship management skills across CCL regulators, Medi-Cal managed care plans, staff, and community partners Preferred / Differentiating
- RCFE Administrator Certificate (active, California CDSS/DSS) — required if role involves RCFE facility oversight; strongly preferred otherwise
- Operational experience across more than one California license type (e.g., RCFE plus ARF, or RCFE plus licensed Home Health Agency)
- Experience with CalAIM Enhanced Care Management (ECM), Community Supports, or ILOS program implementation
- Familiarity with CBAS, MSSP, IHSS Public Authority, or In-Home Care program operations
- Experience supporting corporate acquisition, organizational restructuring, or multi-site integration in California
- Master's degree in Healthcare Administration, Public Health, Business Administration, or related field (MHA, MPH, or MBA preferred)
- ACHE Fellow (FACHE), CCM (Certified Case Manager), or California NHA license
- Bilingual: Spanish, Korean, Mandarin, Armenian, or Tagalog strongly preferred given the Los Angeles service population
- Current DOJ/FBI LiveScan clearance and OIG/LEIE clean record (required prior to hire)
