Job Summary

The AOT Psychiatrist provides expert psychiatric evaluation, medication management, and clinical oversight for individuals enrolled in Lead4Life's Assisted Outpatient Treatment (AOT) Program. Operating within a court-ordered treatment framework as governed by Maryland COMAR 10.21.20, the Psychiatrist serves as the lead medical authority within the interdisciplinary treatment team and bears clinical responsibility for the psychiatric component of each participant's individualized treatment plan. This position requires a compassionate, community-oriented clinician who understands the unique dynamics of serving individuals with serious mental illness (SMI) in a legally structured outpatient setting. The Psychiatrist also provides clinical guidance to program staff and contributes to systemic quality improvement in alignment with Lead4Life's mission of delivering evidence-based, recovery-oriented behavioral health services to underserved Maryland communities.

Duties

Conduct comprehensive psychiatric evaluations and diagnostic assessments for all newly enrolled AOT participants, documenting findings in the electronic health record (EHR) in accordance with COMAR 10.21.20 and Lead4Life clinical standards.
Develop, implement, and regularly review individualized psychiatric treatment plans for each AOT participant, ensuring plans align with court orders and address symptoms, functioning, safety, and recovery goals.
Provide ongoing medication management, including prescribing, monitoring, and adjusting psychotropic medications; document rationale for all medication decisions in compliance with Maryland Board of Physicians standards.
Participate in and provide testimony at AOT court hearings as required, presenting clinical findings, medication compliance status, and treatment recommendations to circuit court judges and legal counsel.
Collaborate with the interdisciplinary care team—including case managers, care coordinators, peer recovery specialists, and outreach staff—to ensure integrated, holistic treatment for each participant.
Provide clinical crisis intervention and consultation for AOT participants experiencing acute psychiatric decompensation, coordinating inpatient hospitalization when clinically indicated and managing timely transitions back to community-based care.
Monitor each participant's compliance with court-ordered psychiatric treatment, including medication adherence and attendance at scheduled appointments; report non-compliance to the Program Director and Court Liaison as required by COMAR 10.21.20.
Provide clinical supervision and psychiatric consultation to LCSW, LMSW, LCPC, and other clinical staff within the AOT team, fostering a culture of clinical excellence and trauma-informed practice.
Maintain complete, timely, and accurate documentation in the electronic health record (EHR) for all clinical encounters, consistent with Maryland Medicaid documentation requirements and COMAR 10.21.20 standards.
Participate in quality assurance and performance improvement activities, including clinical record audits, outcome measure review, and interdisciplinary case conferences.
Coordinate with primary care physicians, hospitals, emergency departments, and specialty providers to ensure seamless medical and psychiatric care integration for AOT participants.
Evaluate and manage co-occurring substance use disorders in conjunction with addiction medicine specialists and the AOT care team, consistent with COMAR 10.21.17 community-based service standards.
Provide psychoeducation to participants and family members regarding diagnoses, medications, side effects, and the AOT process, promoting informed consent and participant engagement.
Ensure all psychiatric services meet or exceed COMAR 10.63 Behavioral Health Administrative Services Organization (BHASA) clinical standards and Lead4Life's accreditation requirements.
Remain current with advances in community psychiatry, psychopharmacology, and evidence-based practices; participate in continuing medical education (CME) and contribute to staff training initiatives.

Experience

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution required
Completion of an accredited Psychiatry residency program required
Board certification in Psychiatry by the American Board of Psychiatry and Neurology (ABPN) required (or board-eligible with commitment to sit within one year)
Active, unrestricted Maryland medical license required
Current DEA registration (Schedule II–V) required
Minimum of three (3) years of post-residency clinical experience in community psychiatry, assertive community treatment (ACT), or forensic/court-involved populations preferred
Experience with electronic health records (EHR) systems required; proficiency in Maryland Medicaid documentation strongly preferred
Subspecialty training or certification in Addiction Psychiatry or Forensic Psychiatry a plus
Bilingual (English/Spanish) preferred

Other Requirements:

Valid driver's license and insurable driving record required
Must pass pre-employment drug screening
Must pass criminal background check (CJIS/FBI) per Maryland law
Must pass Child Protective Services (CPS) clearance
TB test or health screening as required by program
Current CPR/First Aid certification (or willingness to obtain within 30 days of hire)
Physical examination clearance if role involves direct client transport

Pay: $160,000.00 - $180,000.00 per year
