About Us

Nevada Holistic Medicine is dedicated to providing personalized, integrative healthcare that addresses the whole person—mind, body, and lifestyle. Our team combines evidence-based medicine with holistic approaches to help patients achieve optimal wellness and long-term health outcomes. We are seeking a compassionate, organized, and patient-focused Holistic Patient Journey Coordinator to guide patients through every stage of their care experience.

Position Summary

The Holistic Patient Journey Coordinator serves as the primary point of contact for patients from initial inquiry through ongoing treatment and wellness programs. This role is responsible for coordinating appointments, supporting patient engagement, facilitating communication between patients and providers, and ensuring a seamless, supportive experience throughout the patient's wellness journey.

The ideal candidate is highly empathetic, detail-oriented, and passionate about holistic health, preventive care, and patient advocacy.

Key Responsibilities

Patient Onboarding & Engagement

Welcome new patients and guide them through the intake and onboarding process.
Assist patients with completing required forms and documentation.
Educate patients on available services, treatment programs, and wellness offerings.
Provide a warm, supportive, and personalized patient experience.
Serve as the primary non-clinical contact throughout the patient journey.

Care Coordination

Schedule consultations, follow-up visits, wellness assessments, and program appointments.
Coordinate patient care plans between providers, coaches, and support staff.
Monitor patient progress and ensure timely follow-up communications.
Assist patients in understanding treatment schedules and recommended services.
Track patient milestones and engagement throughout care programs.

Patient Communication

Respond promptly to patient inquiries via phone, email, text, and patient portals.
Conduct appointment reminders and wellness check-ins.
Communicate provider recommendations and next steps as directed.
Help patients navigate administrative aspects of their care.
Escalate clinical questions to licensed healthcare providers when appropriate.

Administrative Support

Maintain accurate patient records and documentation.
Update CRM, EHR, and scheduling systems with patient interactions.
Monitor patient pipeline activities and appointment statuses.
Assist with membership programs, wellness packages, and service enrollments.
Ensure compliance with HIPAA and patient privacy requirements.

Retention & Patient Success

Identify opportunities to improve patient engagement and satisfaction.
Encourage adherence to treatment plans and wellness programs.
Follow up with inactive patients regarding recommended care.
Support patient retention initiatives and educational outreach campaigns.
Collect patient feedback and assist with service improvement efforts.

Qualifications

Required

High school diploma or equivalent.
2+ years of experience in patient coordination, customer success, healthcare administration, concierge medicine, wellness services, or client relationship management.
Excellent verbal and written communication skills.
Strong organizational and multitasking abilities.
Ability to manage sensitive patient information with professionalism and discretion.
Proficiency with CRM systems, scheduling software, Microsoft Office, and Google Workspace.
Ability to work independently in a remote environment.
Reliable internet connection and dedicated home office workspace.

Preferred

Experience in holistic, integrative, functional, naturopathic, wellness, chiropractic, or alternative healthcare settings.
Familiarity with patient engagement and retention strategies.
Experience using electronic health record (EHR) systems.
Knowledge of wellness coaching, nutrition, lifestyle medicine, or preventive health concepts.
Bilingual communication skills.

Pay: $27.12 - $32.66 per hour
