We are growing and looking for an organized, compassionate, and highly motivated Patient Care Coordinator to become an essential part of our patient care team.

Position Summary

As a New Patient Coordinator, you will be the first point of contact for many of our patients. You will be responsible for processing incoming referrals, communicating with patients, scheduling appointments, and coordinating care to ensure every patient moves efficiently through the diagnostic and treatment process.

Success in this role requires strong organizational skills, attention to detail, excellent communication, and the ability to manage multiple priorities in a fast-paced healthcare environment.

Responsibilities

Process incoming faxed and electronic referrals promptly and accurately.
Review referrals for completeness and identify missing documentation.
Contact patients to introduce our practice and assist them through the scheduling process.
Schedule new patient consultations, sleep studies, and follow-up appointments.
Coordinate care between referring providers, insurance companies, and internal clinical teams.
Verify demographic information and ensure accurate patient registration.
Monitor referral work queues to ensure timely patient outreach and scheduling.
Document all patient interactions accurately within the electronic medical record.
Follow up on outstanding referrals and incomplete documentation.
Maintain excellent communication with providers, medical assistants, and administrative staff.
Deliver exceptional customer service while maintaining patient confidentiality and HIPAA compliance.
Assist with other administrative duties as needed to support clinic operations.

Qualifications

High school diploma or equivalent required.
Previous healthcare, medical office, scheduling, or referral coordination experience preferred.
Experience with electronic medical records (EMR) is preferred.
Strong computer and data entry skills.
Excellent verbal and written communication skills.
Exceptional organizational and time management abilities.
Ability to multitask and prioritize effectively.
Positive attitude with a strong sense of urgency.
Professional, dependable, and patient-focused.
