Full Job Description

Care Collective is a geriatric care management company supporting older adults and their families through complex, high-touch care coordination. We work with clients navigating cognitive decline, chronic illness, post-hospitalization transitions, and other clinically complicated situations — bringing clinical expertise, compassion, and hands-on coordination to every case.

About the Role:

We're looking for an experienced Registered Nurse to join our team supporting clinically complex geriatric cases throughout Fairfield County, CT. This role is ideal for an RN who wants flexible, meaningful part-time work and thrives on independent clinical judgment, family communication, and coordinating care across multiple providers.

You'll work directly with our care management team on an as-needed/per-case basis, conducting home visits, clinical assessments, and care coordination for older adults with complex medical, cognitive, and psychosocial needs.

What You'll Do:

Conduct in-home nursing assessments for clients with complex medical and cognitive conditions

Monitor and report on changes in condition, medication management, and care plan adherence

Coordinate with physicians, specialists, hospitals, rehab facilities, and family members to ensure continuity of care

Support hospital-to-home transitions, including discharge planning follow-up

Communicate clearly and compassionately with clients and families, often during stressful or emotionally difficult circumstances

Document visits and clinical findings in a timely, thorough manner

Collaborate with our care management team to flag concerns and adjust care plans as needed

Respond to scheduled and occasional urgent visit requests within your availability

What We're Looking For:

Active, unrestricted RN license in Connecticut (required)

Minimum 3+ years of nursing experience; geriatric, home health, hospice, or care management background strongly preferred

Comfortable working independently and exercising sound clinical judgment in a home-based setting

Experience with complex/high-acuity cases: dementia and cognitive decline, chronic disease management, polypharmacy, post-acute care

Strong communication skills — you can translate clinical information clearly for families who are not medical professionals

Reliable transportation and willingness to travel to client homes throughout Fairfield County

Comfortable with a flexible, per-case schedule (this is not a set-hours W-2 role)

CPR certification current

Compensation & Structure

1099 independent contractor position — competitive per-visit or hourly rate, commensurate with experience

Flexible scheduling based on case volume and your availability

Opportunity for ongoing, recurring case work as our client base in Fairfield County grows
