Bilingual Lead Case Manager (Spanish/English) – CalAIM Enhanced Care Management (ECM)

Make a meaningful impact while leading compassionate, person-centered care.

We are seeking a Bilingual Lead Case Manager to join our growing CalAIM Enhanced Care Management (ECM) team. This position is ideal for someone who is passionate about improving health outcomes, enjoys building relationships with members, and is ready to take on a leadership role within a collaborative and supportive team.

As a Lead Case Manager, you will coordinate comprehensive care for Medi-Cal members with complex medical and social needs while serving as a resource and mentor to fellow Case Managers.

Responsibilities:

Conduct outreach efforts to identify and engage eligible CalAIM recipients in need of case management services
Develop and implement individualized care plans tailored to each recipient's unique needs and goals
Coordinate and facilitate access to a wide range of medical, social, and community-based services
Maintain accurate and up-to-date case records and documentation
Collaborate with interdisciplinary teams, including healthcare providers, social workers, and community partners
Advocate for recipients to ensure they receive the necessary support and resources
Provide education and guidance to recipients and their families on available services and programs
Participate in case conferences and team meetings to discuss and review case progress

Qualifications:

Current Licensed Vocational Nurse (LVN) license in good standing HIGHLY PREFERRED
Ability to speak Spanish and English, required
Experience in case management appreciated, however we are able to train
Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations
Proficiency in computer applications and electronic medical record systems
Demonstrated ability to work independently and as part of a team
Organized, detail-oriented, and able to manage multiple tasks and priorities
Compassionate and patient-centered approach to care
Willing to travel across the east and west areas of Los Angeles County
