Aging and Long-Term Services Department's (ALTSD) mission is the belief that New Mexico's older adults and adults with disabilities have the right to remain active participants in their communities, to age with respect and dignity, to be protected from abuse, neglect, and exploitation and to have equal access to health care. The Long-Term Care Division (LTCD) at ALTSD administer community-based programs to allow older adults to thrive in their homes and communities to reduce nursing home placements and emergency department visits.

Why does the job exist?

Under the Direction of the Care Transitions Supervisor, the Care Transitions Specialist will:

Oversee program rules and eligibility
Provide technical assistance and support to participants, caregivers, and contractors
Resolve complex client issues
Track and report performance measures accurately and timely, as established by the agency
Assist the program with other duties as assigned.

How does it get done?

At the direction of the Care Transitions Supervisor the Care Transitions Specialist is responsible for:
Case Management
Recruit participants and determine eligibility Conduct program presentations
Track and monitor participant cases, including care plans and follow-up
Provide in-facility visits and assessments
Connect clients with local resources (financial aid, legal support, healthcare, etc.)
Make necessary referrals and provide support through both virtual and in-person engagement
Identifies, investigates, and resolves complaints on behalf of clients with care providers.
Advocates with New Mexico Manage Care Organizations to ensure clients receive the services they need.
Represents the interests of residents before government agencies and seeks administrative, legal, and other remedies to protect the health, safety, welfare and rights of clients.
Provides follow up care transition services for up to 6 months.
Maintain assigned caseloads

Outreach to Participants and coordination with partners
Collaborate with service providers, state agencies, and community organizations
Advocate for services and assist families in accessing benefits
Stay informed on federal, state, and local benefits and resources
Maintain partnerships to support program goals

Tracking and Reporting.
Receive, enter and document referrals
Maintain accurate and timely case records
Who are the customers?

ALTSD clients who are older adults and adults with disabilities who have complex needs and require long-term care services and supports with care transitions, other staff agency, contractors, community organizations.
Ideal Candidate

Minimum Qualification

Associate's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, Family Studies/Services, Social Sciences, or Human Services and one (1) year of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling three (3) years may substitute for the required education and experience.
Employment Requirements

Must possess and maintain a valid Driver's License. Background and business reference check and Driver's License verification are required, and employment is conditional pending results.
Must possess and maintain a current Defensive Driving Course certificate from the State of New Mexico or must pass and receive a Defensive Driving Course certificate as a condition of continued employment.
Working Conditions

Work is performed in an office setting, long-term care facility setting, and community setting with extensive use of the personal computer, telephone, fax,, copier and other related office equipment. Occasional after hours and occasional weekends may be required. Statewide travel is required.
