Community Health Worker (CHW)

At IMPaCT Care, we specialize in partnering with healthcare organizations to recruit and hire community healthcare workers. We are currently hiring for a few opportunities with (Kaiser Permanente & IMPaCT Care) to fill these exciting new positions. If hired you will work directly for IMPaCT Care.

This is a full-time position with IMPaCT Care. You will be a direct employee of IMPaCT Care, working embedded within our Kaiser Permanente partnership in San Bernardino, CA.

ABOUT THE ROLE

As a Community Health Worker, you’ll be a vital link between high-risk patients and the care and services they need. You’ll do this through trust, cultural understanding, and hands-on problem-solving — working in the community, not behind a desk.

WHO YOU ARE

You are someone your community already knows and trusts — a neighbor, a peer, an advocate who has walked the path your clients are on. That lived experience is the foundation of this work. It's what opens doors that credentials alone cannot.

But trust gets you in the door — skills are what make the difference once you're there. You also bring real working knowledge of community health, social services, or peer support. You know how to navigate systems, coordinate care, and follow through in ways that actually move the needle for clients.

The combination of both is what makes an exceptional CHW.

WHAT YOU’LL DO

Build relationships & deliver care

Build trust through consistent presence and culturally aligned, empathetic communication
Identify and address barriers to care: food insecurity, transportation, housing, and more
Connect patients to care teams and community resources; follow up to ensure needs are met
Document interactions, insights, and emerging patient needs using digital tools
Use feedback and performance data as tools for growth, actively refining your approach based on what the metrics show about client outcomes
Document all client interactions, needs, assessments, and outcomes in EPIC EMR System
Complete SDOH (Social Determinants of Health )screenings and update care plans in a timely mannerParticipate in integrated care huddles and collaborate with clinical teams

WHAT YOU BRING

Required

High school diploma or equivalent
1–3 years in community-based outreach, health education, or related work
Valid driver's license, must own a vehicle, have car insurance
Ability to pass background check and pre-employment screening
Field based work

Preferred

Bilingual Spanish preferred
Experience with an EMR system such as Epic,
Familiarity with Medicaid, D-SNP, waiver programs, and recertification cycles
Experience with SDOH (Social Determinants of Health ) screening tools (e.g., Hunger Vital Sign, USDA Household Food Security) or case management platforms
Knowledge of HEDIS (Healthcare Effectiveness Data and Information Set) metrics or risk stratification concepts
Strong ties to community organizations: faith institutions, shelters, food banks, or mutual aid networks

WHAT TO EXPECT: YOUR DAY-TO-DAY This section is new

This is a field-based position. You will spend the majority of your time in the community — in clients’ homes, in hospitals, in clinics, and at community organizations. You will report regularly to Kaiser Permanente locations for team meetings, training, and care huddles.

A typical week might include:

Client home or community visits
Care huddle with the clinical team at partner site
Documentation and care plan updates
Phone and text outreach to clients between visits

Schedule: Monday through Friday, 9am–5pm. Field based work.

PRE-EMPLOYMENT REQUIREMENTS

Background check
Pre-employment drug screen
Health and medical clearance, including proof of required vaccinations from a primary care MD
Valid driver’s license and reliable personal vehicle; ability to travel across an assigned region to meet patient needs at offices, clinics, and patient homes

TB screening (skin test, blood test, or chest x-ray depending on history)
Hepatitis B
Measles (Rubeola)
Mumps
Rubella
Varicella
Pertussis / Tdap (Whooping Cough)
Influenza (required August 1 – April 30)
