Full Job Description

1. Evaluation and assessment of a child for placement and continued placement in a certified family home or licensed foster family home and Resource Families.

2. Placement of the child in the certified family home or licensed foster family home.

3. Meet with foster parents as required to comply with ongoing documentation, family therapy, supervisions, training, client’s records maintenance, and certification requirements as required by Building Bridges FFA, County Contract and CCL.

4. Ongoing evaluation and assessment of a child for placement and continued placement in a certified family home r licensed foster family home.

5. Placement of the child or nonminor dependent in the certified family home, licensed foster family home, or Resource Family.

6. Development and updating of the child’s needs and services plan, including identifying individuals who are important to the child are required by Welfare and Institutions Code Section 366.1(g). Develop and consistently implement the Needs and Services Plan for children under his/her care in collaboration with foster parents, county workers, and birth parents or legal guardians (when possible).

7. Supervision of the placement of the child including direct contact with the child and the certified parent(s) or foster family home licensee(s).

8. Provision of support services to certified parents or foster family home licensees and Resource Families assigned to the (social worker.

9. Meet medical, dental, educational, religious, recreational, family, social, emotional, and psychological needs of each child on caseload in conjunction with available community Fosters.

10. Maintain casework, medical, and home files on individual caseload; prepare all required reports thoroughly and in a timely manner for these files.

11. Submit required reports to state licensing and placing agency in a timely manner.

12. For each Foster home on his/her caseload, monitor adherence to licensing regulations. This includes driving to each home and physically surveying and supervising the care and treatment of each child in the home, as minimum, once per week.

13. Obtain and provide foster families with required admission information upon initial placement.

14. Develop and maintain network of medical, dental and psychological care for foster children in the area of placement.

15. Monitor current status of educational planning and academic functioning of each foster child on his/her caseload. Represent the child in educational settings whenever necessary.

16. Participate in assessment team meetings in order to discuss individual cases, and to develop treatment goals.

17. Support foster families via training, coaching, and being available 24 hours/7 days a week.

18. Assume responsibility for maintaining all licensing and/or agency requirements or certifications, contractual agreements and/or accreditations.

19. Actively promote the best interest of the agency both internally and externally whenever possible.

20. Monitor continuously the safety and stability of the Foster home.

21. Report any actual or suspected child abuse by way of special incident reports to Building Bridges FFA and appropriate authorities. Participating in the investigations involving complains or allegations regarding any deficiencies and violations of regulations, applicable laws as well as Building Bridges FFA program guidelines and procedures up to decertification of any foster family home.

22. Maintain adequate office coverage during business hours to respond to any foster home calls as needed and required by Building Bridges FFA, County and CCL. Social worker personnel will complete and submit a weekly schedule to Social Worker Supervisor.

23. Complete other assignments as required by supervisor.

24. Participate in BBFFA events (Christmas Party, Summer Picnic etc.), including planning, preparation and set-up of such events.

25. Attend recruitment events as required.

26. Attend monthly Social Worker meetings and quarterly all staff meetings.

27. Participate in monthly foster parent in service training as required.

28. Be available in the office during duty days.

29. Respond to emergencies involving assigned families and complete incident reports in a timely manner.

30. Submit timesheets by the due date established by the Accounting and/or HR department.

31. Submit weekly planner and advise supervisor and/or administrator or changes to anticipated schedule.

32. Attend IEPs, CFTs, TDMs and other meetings for assigned children.

33. Submit case notes within 1 week of visit. Submit reports and other documents on or before the due date.
