Job Description – QDDP
Job Summary: The QDDP is responsible for providing a comfortable and normative environment in the home and/or community. Employees should support the person to have the opportunity to develop to the fullest extent of his/her abilities and desires, while ensuring the person feels safe and loved. They will establish a warm and personal relationship with each person they are supporting and encourage active participation in the community. They will also encourage responsible decision making. Supervises an assigned program and/or employees.

Duties and Responsibilities:
With the person:
- Get to know the people supported by the Agency and build relationships in positive ways.
- Support the person to feel safe and loved while they are with you.
- Support the person to develop connections and relationships.
- Provide a comfortable and normative environment in the home and/or the community.
- Encourage/Role Model the skills each person needs supports with. Document, implement and evaluate supports as required and requested. Keep all Books up to date.
- Implement all support Plans, protocols, policies and procedures, while ensuring safety as a priority.
- Monitor health needs ensuring that preventative health care practices are utilized, including encouraging healthy lifestyles. Also, ensure immediate treatment for illness or accident. Ensure all annual medical appointments are scheduled and completed, including routine labs, as well as any follow-up appointments as recommended. Ensure all documentation is in the Home Book.
- Ensure the administration and recording of medications in accordance with Agency policies, including what to do if medication is refused. Ensures that medications are ordered, such as PRN and non-cycle medications, as needed. Ensure all PRN medications are not out of date. Review cycle medications and orders prior to the beginning of each month. Ensure all new orders are transcribed to the POS and the MAR as soon as the prescription is written. Also ensure the medication is ordered from the pharmacy. Ensure controlled substances are being counted and document on the Controlled Substance Count Sheet. Notify the RN if any medications need to be destroyed.
- If a person we support has to go to the emergency room the QDDP will make sure all required paperwork (Medical/Dental Detail, Physicians Orders) are completed and copies are made for the Home Book, and the Community RN, with the original going in the Case Record. Ensure proper ratios are met at the home. If a person we support is admitted to the hospital, the QDDP will ensure someone stays with the person until they are settled in the room and all important information is shared with the hospital. The QDDP will ensure the guardian, Executive Director, KCRO and Support Coordination Agency on-call are all notified. Then ensure Event Report is completed. After all unusual incidents occur including, falls, restraint, emergency room visits, etc, ensure a Post- Event Protocol is completed.
- Work with all individuals and ensure a high standard of personal care is maintained, in a respectful manner, while also being a role model in this area.
- Make available to all people the opportunity to participate in integrated community activities. Assist the person to learn new skills, make responsible choices and be involved in what is happening in their lives. All this is to be accomplished while showing the person dignity and respect.
- Assist the person to organize and pack if the person moves. Ensure the Move- In packet is completed when the person moves, including personal inventory. Ensure all required documentation is obtained when we start supporting a new person, including; immunization records, required medical documentation, signed Physician’s Orders, original cards, releases, guardianship paperwork, etc. If the previous Agency or person does not get these to us, QDDP must ensure they are obtained as soon as possible.
- Assist/teach/role model each person with self-care, budgeting, banking, cooking, household management, socialization, shopping and recreation as required.
- Assist each person we support to complete and sign all paperwork required to continue services, ensure insurance is billed for medical services and ensure all paperwork is obtained as required by licensure and certification for the state within the time frames allotted.
For the Agency:
- Take on-call responsibilities as assigned. Return all calls from the answering service/on-call phone promptly. Ensure the person on-call is completely informed as to all concerns that may occur at each home including making sure a schedule and updated information is given to the person on-call. On Friday an updated schedule with Staffing Agency staff identified must be to the Weekend Supervisor by 3pm. Review all documentation contained within the On-Call Book. Ensure documentation is current. If any information is not current obtain the information and file it promptly.
- Reply to all situations in accordance with Agency policy and procedures and all company standards.
- Ensure schedules are complete at least on a weekly basis, preferably monthly. Ensure staff follow company policy when taking off. If an employee cannot find appropriate coverage for their own shift, QDDP should assist in finding coverage. QDDP may be required to work a shift if no other coverage is found in order to meet staffing ratios. Ensure all documentation is obtained prior to an employee covering a shift. Approving Day Off Requests. Vacation Requests must go through Human Resources Manager in order to ensure the employee has vacation time to take. Document on Absentee Calendar all absences and tardiness for each employee supervised. Submit Day Off Request, Open Shift Agreements and Vacation requests to The Human Resource Manager. Limit the amount of over-time an individual employee receives, always keeping the Executive Director informed as to the over-time accumulation. If no appropriate employee is available for coverage the QDDP will work the shift in order to maintain all required ratios. Ensure additional employees are scheduled when needed, i.e. Royals games, Starlight.
- Ensure each employee supervised receives a yearly evaluation. All pay increases must be submitted to the Office Manager prior to the date they should take effect. Review the evaluation with the employee prior to the date of any pay change. Ensure Change of Status paperwork gets completed as required. Ensure change of shift is documented in Employee Personnel Record on the Personnel Tracking Form. Have corrective discussion with employees as the need arises. Ensure documentation of all Corrective Action is obtained and submitted to the Human Resource Manager. Ensure Exit Interview is completed with accuracy and submitted to the Human Resource Manager. Nominate employees for Employee of the Month.
- Home visits/contact required at least once per week for each home receiving supports. Review the Plans and know the information contained with-in. Document on Home Visit form, event reports and any other documentation that is appropriate. Ensures Support Employees document on all appropriate paperwork. If an Event Report/ Med Report is completed, review the document to ensure it is completed accurately. Review the monthly packet when it is submitted in order to ensure accuracy of all information. Ensure all documentation is complete and accurate prior to filing in the Case Record. Back pull and back file any information a year old in the Home Book and Case Record Book. Ensure all pertinent information stays in the Book; immunization records, Provider Statement, copy of birth certificate, etc.
- Responsible for proper financial record keeping and accounting for certain funds (PSA, grocery, household, checking accounts). Also, ensuring all monies are accurate and accounted for at all times, with receipts for all purchases. QDDP should verify the accounts when in the home. Request additional funds from Support Coordinator when needed. Ensure all monies get spent in order to keep funds under state required amounts.
- Record and read all relative information in the appropriate log and communication books when in the home. Complete all required documentation, ensuring the confidentiality of the person supported, employees and Building on Dreams and reports.
- Fill out Monthly Planning Meeting Form prior to the monthly meeting. Ensure Monthly Planning Meeting Forms are completed for all meetings within the home. Schedule, facilitate and attend all Plan reviews, house meetings, employee development, and management meetings. Attend all in-service trainings as required. When required develop the Plan, always give input during the planning process. Ensure all required information is contained within the Plan. Implement all Plans within the home. Develop outcome data sheets. Ensure all outcome documentation is being done according to the signed Plan including; frequency, duration, etc. All PCM data must be submitted to the Executive Director for the given quarter by the 2ndbusiness day of the following month. (Example: January, February and March data must be submitted by the 2ndbusiness day in April)
- Review all RN Monthly Assessments, sign and follow up with any concerns expressed by the RN. Ensure Support Coordinator signs off on RN monthly Assessments. Review annual Choking Assessments as they are completed. Submit to physician if any precautions are found, physician should evaluate if a swallow study would be appropriate.
- To maintain good communication with families, other employees, people supported, community agencies, in conjunction with the Executive Director and submit paperwork as required. Assist the person to develop natural supports within the community. Continue to serve as a liaison with funding sources and all community resources.
- Ensure Emergency Preparedness are done on rotating shifts monthly with at least 2 overnight drills a year. Overnight drills must be done by the person working the overnight shift, in the middle of the night.
- Coordinate and assist, when necessary, in training employees, including Staffing Agency, and ensure Occupational Checklists, including reading the Plan, are completed by a qualified employee. Ensure qualified employees are trained to train others within each home. Ensure 3 medication pass observations are completed prior to passing medications independently. Create Occupational Training Checklist when we start supporting a person we have not supported before. Update OTC as information changes.
- Ensure billing is submitted to the Executive Director for the current month 3 business days before the end of the month. Notify the Executive Director immediately if the previously submitted billing is inaccurate. Billing includes; exception hours, Day-Hab Budget Sheets, Ja-Code and absentee (for the people we support) calendars.
- Ensure completion of paperwork for Early Entry/Early Exit and PTO, after receiving permission from supervisor. Document all time working on person specific tasks in QDDP Logs. Ensure required Q hours are completed for each person supported.
- All other duties as assigned.
Qualifications
- Bachelor’s Degree.
- Must pass abuse and criminal record screening.
- Must have valid driver’s license and current car insurance.
- Must have 3 positive references- at least 2 from previous employers.
- Vehicle available for use while working

- Must possess the skills that the person being supported is working on, i.e., if a person needs support with cooking, the person must know how to cook.
- There may be other specific qualifications depending upon the needs and wants of the person, i.e. if the person is hearing impaired, the employee may need to know sign language.
Additional Information:
This position may require transportation of the person we support in a personal vehicle. This position may be required to work in a stressful environment, working with people in crisis situations. The ability to function independently and under pressure is an ongoing expectation of this position. The QDDP position requires special attention to organization and time management. It is extremely important for all QDDP’s to possess these skills. QDDP should understand the needs of each person we support come’s first. Because of this, the QDDP may have to work long hours to ensure completion of all required duties.
