Hospice RN Case Manager (Full-Time)

Location: Riverside, CA and surrounding service areas

We are seeking an experienced, compassionate, and dedicated Hospice RN Case Manager to join our growing team.Our agency is dedicated to providing exceptional, compassionate end-of-life care while fostering a positive and supportive work environment built on professionalism, accountability, compliance, and teamwork. Position Details

Position: Full-Time
Compensation: Rate discussed during the interview
Start Date: Able to start ASAP
Service Area: Riverside and surrounding cities
Travel: Must be willing and able to drive throughout the agency's coverage area.

Qualifications

Current, unrestricted California Registered Nurse (RN) license.
Minimum of 2 years of recent hospice experience as a Hospice Visiting Nurse or Hospice RN Case Manager.
Current BLS/CPR certification.
Valid California driver's license, reliable transportation, and current automobile insurance.
Knowledge of hospice regulations, CDPH requirements, Medicare Conditions of Participation, and hospice documentation standards.
Proficiency with EMR documentation and strong clinical assessment, communication, organizational, and time management skills.
Ability to work independently while collaborating effectively with the interdisciplinary team.

Essential Duties & Responsibilities

Provide skilled nursing care and case management to hospice patients in accordance with the individualized plan of care.
Complete comprehensive assessments, monitor patient status, and coordinate care with physicians and the interdisciplinary team.
Provide pain and symptom management while educating and supporting patients, families, and caregivers.
Maintain accurate, timely, and compliant clinical documentation for all patient care and coordination activities.
Participate in Interdisciplinary Group (IDG) meetings and contribute to ongoing care planning.
Supervise Hospice Aides and LVNs as assigned.
Maintain compliance with all applicable agency policies, CDPH regulations, Medicare Conditions of Participation, HIPAA, and quality standards.
