Full Job Description

Schedule: 35 hours/week | Full-time | Hourly

Job Summary

This position provides person-centered case management services to individuals returning to the community from incarceration, as part of Delphi Rise’s Monroe County Reentry Program. Our approach is grounded in dignity, respect, and belief in each person’s potential. We walk alongside clients as they rebuild their lives, offering support with housing, employment, health care, and healing.

At Delphi Rise, we don’t define people by their past. We focus on building trusted relationships that foster growth, reduce barriers, and strengthen community connections because we know that no one can do it alone. Through our reentry program, we support individuals in creating a future defined by stability, purpose, and hope.

Essential Duties and Responsibilities

Additional duties may be assigned.

Manage a caseload of 40 to 50 clients, supporting access to benefits, employment, treatment, housing, childcare, healthcare, identification documents, and other critical services.

Coordinate services in collaboration with the New York State Department of Corrections and Community Supervision (Parole), Monroe County Probation, local courts, and community partners.

Maintain accurate and timely case notes and client records.

Develop individualized service plans that align with each client’s goals and needs.

Facilitate or arrange support groups such as Cognitive Behavioral Therapy, parenting education, domestic violence support, and anger management.

Conduct biweekly follow-up contacts with all assigned clients.

Actively participate in Monroe County Reentry Task Force meetings.

Provide services on-site at the Parole Office one to two days per week.

Perform other duties as assigned by management.

Supervisory Responsibilities

This role does not include supervisory responsibilities.

Knowledge, Skills, and Abilities

Ability to build rapport and effectively engage individuals with serious mental health challenges or criminal justice involvement.
Strong written and verbal communication skills.
Excellent organizational skills with the ability to manage multiple priorities.
Analytical thinking to assess needs, prioritize actions, and solve problems.
Familiarity with community resources that meet diverse client needs.
Ability to work independently while contributing positively to a team environment.
Cultural humility and comfort working in diverse environments.
Adaptability and a good sense of humor.
Proficiency with computers, including web-based data systems.
Valid driver’s license with a clean driving record. Occasional travel required for meetings, appointments, and community events.

Equipment Used

Regular use of computers/iPads, printer, copier, scanner, fax, phone, shredder, and calculator. Software includes Microsoft Office: Word, Excel, Outlook, PowerPoint, and Access.

Mental and Visual Demands

Moderate attention required for administrative tasks, data entry, and client documentation. Some analysis and decision-making needed within established procedures.

Physical Requirements

The role involves frequent sitting, standing, walking, talking, hearing, and repetitive hand motions. Occasional pushing or pulling. No heavy lifting (under 5 pounds).

Environmental Conditions

Some off-site work may be required to meet with clients or attend community events. This may include exposure to inclement weather or travel to partner agency locations.
