Full Job Description

Job Summary
We are seeking an experienced compassionate and dedicated Registered Nurse for our Home Health Agency. The ideal candidate will provide high-quality patient care in a home setting, ensuring adherence to established standards and protocols. This role requires strong clinical skills, excellent communication abilities, and a commitment to patient-centered care. The Registered Nurse will work closely with patients, families, and interdisciplinary teams to promote optimal health outcomes.

Duties

Conduct comprehensive assessments of patients’ health status and needs.
Develop and implement individualized care plans based on patient assessments.
Administer medications and treatments as prescribed by healthcare providers.
Monitor patients for changes in condition and respond appropriately.
Provide education to patients and families regarding care plans, medications, and health management.
Ensure compliance with infection control standards and HIPAA regulations.
Assist with pain management strategies to enhance patient comfort.
Offer support in basic life support techniques when necessary.
Collaborate with other healthcare professionals to coordinate care for patients with acute or chronic conditions.
Maintain accurate documentation of patient care activities and progress.

Requirements

Current Registered Nurse (RN) license in the state of practice.
Proven experience in home health care or a similar setting is preferred.
Knowledge of infection control practices and standards of care.
Familiarity with pain management techniques and patient care protocols.
Understanding of HIPAA regulations to ensure patient confidentiality.
Experience in caring for patients with various ailments, including infant care, acute care, and inpatient settings is advantageous.
Strong critical thinking skills and the ability to make sound clinical decisions.
Excellent communication skills, both verbal and written, for effective interaction with patients, families, and team members.
Ability to work independently while managing multiple priorities effectively.
