GENERAL POSITION SUMMARY
Our Homeless Veterans Reintegration Program (HVRP) assists veterans who are homeless, or at risk of becoming homeless in achieving employment. The HVRP Case Manager provides comprehensive case management services to homeless veterans, supporting them in overcoming barriers to employment and achieving long-term stability. This role involves conducting assessments, developing individualized service plans, coordinating supportive services, and guiding participants through job training and placement opportunities. The Case Manager works collaboratively with community partners, employers, and support agencies to connect veterans with resources such as housing, mental health services, and benefits. The Program Coordinator, Project ELEVATE reports to the SCSEP Program Manager and works closely with the Career Navigator. As a team they will communicate with internal and external stakeholders in the furthering of SCSEP Elevate program initiatives.
ESSENTIAL DUTIES AND RESPONSIBILITIESThe following core responsibilities define this role. Additional duties may be assigned as needed.
Outreach & Participant Engagement
- Conduct outreach to shelters, food banks, VSOs, and other partners to recruit eligible veterans.
- Build and maintain relationships with WorkSource Oregon, WIOA providers, and DVOP specialists.
- Represent the program at job fairs, community events, and resource fairs.
Case Management & Employment Support
- Assess eligibility, complete intakes, and enroll participants per ESO and DOL-VETS guidelines.
- Develop Individual Employment Plans (IEPs) with participants, identifying strengths, barriers, and employment goals.
- Connect veterans to resources for housing, transportation, childcare, and healthcare.
- Provide job coaching, resume support, and career counseling.
- Work with community partners and internal resources to remove barriers of employment through support services.
Employer & Community Partnerships
- Establish partnerships with employers, training programs, and community agencies to expand job opportunities.
- Advocate for participants by coordinating access to employment, training, and support services.
- Co-facilitate weekly job club, develop curriculum, and coordinate guest speakers.
Compliance & Data Management
- Maintain accurate case notes and track services in Salesforce and VGRS.
- Ensure compliance with ESO policies and DOL-VETS regulations.
- Attend staff meetings, professional development activities, and required DOL-VETS trainings.
Employer & Community Partnerships
- Establish partnerships with employers, training programs, and community agencies to expand job opportunities.
- Advocate for participants by coordinating access to employment, training, and support services.

EDUCATION and/or EXPERIENCE
- Bachelor's degree in human development or social services, psychology, or a related field, or a minimum of 2 years of case management experience preferred.
- Excellent written and verbal communication skills.
- Demonstrated interest in working with veterans and the ability to be compassionate and understanding.
- Must possess a valid Driver’s License.
- Able to drive throughout Douglas & Coos counties.
- Working knowledge of career planning.
- Experience with Microsoft Word, Excel, PowerPoint, email, and the Internet.
