Full Job Description

We are hiring a Home Health RN in Battle Creek, MI.

Location: Battle Creek, MI
Service Area: Calhoun, Kalamazoo & Van Buren Counties
Contract: 13 Weeks
Shift: Days
Schedule: 5x8-hour shifts
Hours: 8:00 AM – 5:30 PM
Guaranteed Hours: 40 hours/week
Weekend Requirement: Every 4th weekend / approximately 1 weekend per month
Call: None
Experience: Minimum 1 year
Travel Experience: Not required — First-time travelers accepted

Position Overview

We are seeking an experienced Home Health Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. The RN will conduct home visits throughout Calhoun, Kalamazoo, and Van Buren Counties.

The ideal candidate should be comfortable working independently in a home-based environment, performing comprehensive patient assessments, coordinating care, providing patient education, and managing a variety of clinical needs.

Key Responsibilities

Provide skilled nursing and home health care in patients' homes.
Perform comprehensive patient assessments and develop individualized plans of care.
Complete Start of Care (SOC), recertification, and follow-up visits as applicable.
Provide medication management, patient education, and disease management.
Perform wound assessment and wound care as needed.
Monitor patient conditions and identify changes requiring physician intervention.
Coordinate care with physicians, Physical Therapy, and other interdisciplinary team members.
Provide care coordination and case management for assigned patients.
Maintain accurate and timely clinical documentation and EMR records.
Educate patients and caregivers regarding medications, treatment plans, safety, and disease management.
Promote patient independence, safety, and continuity of care in the home setting.
Travel between assigned locations within Calhoun, Kalamazoo, and Van Buren Counties.

Required Qualifications

Active Registered Nurse (RN) license.
Minimum 1 year of nursing experience.
Previous Home Health or home-based care experience preferred.
AHA BLS/BCLS certification required.
Strong patient assessment and clinical judgment skills.
Ability to work independently and manage a daily visit schedule.
Strong communication, documentation, and care coordination skills.
Must be comfortable traveling throughout the assigned service area.
First-time travelers are welcome.
