Clinical Supervisor - Home Health (RN)

As a Clinical Supervisor, you’ll work alongside our Client Service Managers and operations team in our Bensalem office - coordinating care across the full continuum and serving as the clinical backbone of our field nursing team.

This is a role for a Registered Nurse who is ready to bring their clinical expertise to a broader stage - influencing patient outcomes, supporting nurses in the field, and building relationships with the providers, insurers, and care teams that surround each patient.

Position Overview

As a Clinical Supervisor, you’ll be primarily office-based, working in close collaboration with Client Service Managers and operational staff to coordinate care between our internal team and external partners - including insurance providers, primary care physicians, hospitals, and care coordinators.

You’ll build and manage care plans based on physician orders, conduct patient assessments at least every two months, and join sales visits with managers to meet new patients entering skilled services. You’ll also play an active role in the training and development of our field nursing staff alongside our education department.

Responsibilities

Coordinate care for assigned pediatric and adult patients from admission to discharge, including comprehensive ongoing assessments at minimum every 60 days
Collaborate daily with Client Service Managers and operations staff to ensure seamless internal care coordination
Communicate and coordinate with external providers including insurance companies, primary care physicians, hospitals, and care coordinators
Develop and manage individualized care plans based on physician orders
Conduct sales visits alongside managers to newly referred patients entering skilled nursing services
Supervise, support, and educate field nursing staff - ensuring they feel connected, competent, and backed by the office team
Partner with the education department on onboarding, training, and ongoing clinical education for field nurses
Perform on-site supervisory visits to assess patients, families, environments, and clinical caregivers with complete follow-up documentation
Evaluate quality of nursing practice and identify opportunities to improve patient outcomes and field performance
Ensure all nursing practice aligns with physician orders, agency policy, and state regulations
Serve as a patient and family advocate throughout the care journey

Qualifications

Active PA or Compact Registered Nurse (RN) License - REQUIRED
Diploma, Associate, or Bachelor’s degree in Nursing from a state-accredited RN program - REQUIRED
Valid driver’s license and acceptable MVR - REQUIRED
Minimum one year of nursing experience - REQUIRED
Previous pediatric or home health experience - preferred
Previous case management experience - preferred
Supervisory or leadership experience - preferred, but not required

Skills & Abilities

Strong interpersonal and communication skills - comfortable working across internal teams and external stakeholders
Organized and detail-oriented with the ability to manage a varied caseload
Effective problem-solver with sound clinical judgment
Ability to remain calm and professional under pressure
Comfortable working with diverse pediatric and adult patient populations
Knowledge of compliance, regulatory standards, and professional nursing practice
Commitment to clinical excellence and patient-centered care
Must read, write, speak, and understand English
Must maintain confidentiality and professional boundaries at all times

Work Setting

This is a primarily in-office role based out of our Bensalem, PA location, with flexibility built in. You’ll spend the majority of your time working alongside our clinical and operations team, with periodic field visits for patient assessments and supervisory visits.

Occasional sales visits to prospective patients with skilled services
May include exposure to varied environments, allergens, bodily fluids, or infectious diseases during field visits
