Full Job Description

Overview

Join our compassionate hospice team as an Intake Coordinator, where your organizational skills and empathetic communication will play a vital role in providing exceptional patient care. In this dynamic office-based position, you will serve as the first point of contact for patients and their families, ensuring a smooth and efficient intake process. Your expertise in medical administrative support and knowledge of health regulations will help facilitate timely admissions and coordinate care seamlessly. This role offers an opportunity to make a meaningful difference in the lives of those facing life-limiting illnesses while working in a supportive, team-oriented environment.

Responsibilities

Act as the primary contact for new patient inquiries, guiding families through the intake process with professionalism and compassion
Collect and verify patient information, including medical history, insurance details, and eligibility criteria, ensuring accuracy and completeness
Utilize patient management software and Electronic Health Records (EHR) systems to document all intake activities accurately and efficiently
Coordinate with healthcare providers, insurance companies, and internal teams to facilitate timely admissions and ensure compliance with health regulation policies
Maintain strict confidentiality of patient information in accordance with HIPAA regulations and organizational standards
Answer phone calls promptly, providing clear information about services, scheduling appointments, and addressing inquiries with customer service excellence
Support medical administrative tasks such as data entry, typing reports, managing documentation, and maintaining organized records

Qualifications

Proven experience working in a medical office environment or healthcare setting, preferably with hospice or palliative care services
Proficiency in Microsoft Office Suite (Word, Excel, Outlook) for documentation and communication purposes
Strong understanding of health regulation policies, insurance procedures, and medical terminology
Experience with medical administrative support tasks including patient data entry and managing electronic health records (EHR/EHR systems)
Familiarity with patient management software and electronic health records (EHR) systems to streamline intake processes
Excellent customer service skills with the ability to communicate empathetically via phone and client interactions
Knowledge of health insurance processes to assist with coverage verification and billing inquiries
Ability to maintain patient confidentiality diligently while managing sensitive information
Strong typing skills for accurate data entry and documentation
Office experience demonstrating organizational skills, attention to detail, and multitasking abilities
Bilingual - Spanish Preferred
