Job Description

Sacred Mission Hospice is seeking an experienced and detail-oriented Hospice Quality Assurance (QA) & Compliance Nurse to support our clinical compliance and medical record review processes. This full-time position is ideal for a healthcare professional with hospice documentation, regulatory compliance, and chart auditing experience. The QA Technician will review patient records, clinical documentation, and operational processes to ensure compliance with Centers for Medicare & Medicaid Services (CMS), state regulations, hospice Conditions of Participation, and accreditation standards while supporting our commitment to delivering exceptional end-of-life care.

On an Average Day You Will:

(includes but not limited to)

Perform quality assurance reviews of hospice patient charts and clinical documentation
Audit admission, recertification, discharge, bereavement, HOPE, and HIS documentation for accuracy, timeliness, and completeness
Review physician orders, certifications, plans of care, and interdisciplinary documentation
Ensure compliance with CMS, state, CHAP accreditation standards, and hospice Conditions of Participation
Identify documentation deficiencies and provide corrective recommendations
Track compliance findings and prepare QA reports for leadership
Assist with process improvement initiatives and policy compliance reviews
Collaborate with clinical leadership, including DONs, administrators, and interdisciplinary team members
Support preparation for surveys, audits, and accreditation reviews
Utilize MatrixCare EMR, QAPI Plus, and other electronic medical record systems to review and monitor documentation compliance
Ensure compliance with HOPE submission requirements and regulatory standards
Provide occasional support for routine home care overflow and nurse on-call needs as necessary
Maintain confidentiality and accuracy when handling patient records and sensitive information

Ideal Candidate Meets the Following Requirements:

Minimum of 2 years of hospice, home health, or healthcare compliance experience
Strong knowledge of hospice regulations, documentation standards, Medicare requirements, and Conditions of Participation
Experience reviewing EMR and clinical records with strong familiarity using MatrixCare EMR and QAPI Plus preferred
Strong familiarity with electronic medical record systems and hospice documentation workflows
Working knowledge of HOPE submission processes and compliance requirements
Excellent organizational, analytical, and time management skills
Strong attention to detail with the ability to meet deadlines
Proficiency with Microsoft Office applications
Licensed Nurse (RN/LVN), Medical Records Specialist, or healthcare compliance background preferred
Experience with hospice QA, QAPI, or clinical auditing preferred
Familiarity with CHAP accreditation standards preferred
Ability to work independently and collaboratively in a fast-paced healthcare environment
Willingness to assist with routine home care overflow and nurse-on-call needs on occasion
