Home Health Physical Therapist (PT) Job Description

Job Title: Home Health Physical Therapist (PT)

Reports To: Director of Clinical Services / Rehabilitation Manager

Position Summary:
The Home Health Physical Therapist is responsible for providing skilled physical therapy services to patients in their homes in accordance with physician orders, agency policies, Medicare Conditions of Participation, and state and federal regulations. The Physical Therapist evaluates patients, develops individualized treatment plans, implements therapeutic interventions, and educates patients and caregivers to maximize independence, improve functional mobility, and enhance overall quality of life.

Essential Duties and Responsibilities

Perform comprehensive physical therapy evaluations to assess functional limitations, strength, balance, mobility, gait, pain, and safety within the home environment.
Develop individualized treatment plans based on patient needs, physician orders, and evidence-based practice.
Provide skilled therapeutic exercises, gait training, transfer training, balance training, endurance training, neuromuscular re-education, and manual therapy as appropriate.
Assess home safety and recommend adaptive equipment, durable medical equipment (DME), and home modifications to reduce fall risk and improve independence.
Educate patients, families, and caregivers regarding disease processes, home exercise programs, safety precautions, and fall prevention strategies.
Reassess patient progress regularly and modify treatment plans as clinically indicated.
Complete all required OASIS assessments, progress notes, reassessments, recertifications, discharge summaries, and other clinical documentation accurately and within required timeframes.
Collaborate with physicians, nurses, occupational therapists, speech therapists, medical social workers, home health aides, and other members of the interdisciplinary team to coordinate patient care.
Participate in case conferences and interdisciplinary team meetings.
Communicate significant changes in patient condition promptly to the physician and appropriate team members.
Ensure compliance with Medicare regulations, state practice acts, agency policies, infection control standards, HIPAA requirements, and accreditation standards.
Maintain productivity expectations while providing high-quality, patient-centered care.
Participate in quality improvement activities, continuing education, and competency programs.

Qualifications

Graduate of an accredited Physical Therapy program.
Current, unrestricted Physical Therapist license in the state of practice.
Current CPR/BLS certification.
Valid driver's license, reliable transportation, and proof of current automobile insurance.
Minimum of one (1) year of clinical experience preferred; home health experience is highly desirable.
Knowledge of Medicare home health regulations, OASIS documentation, and PDGM preferred.
Strong clinical assessment, communication, organizational, and documentation skills.
Ability to work independently while maintaining collaboration with the interdisciplinary team.

Physical Requirements

Ability to travel between patient homes throughout the service area.
Ability to stand, walk, bend, kneel, lift, transfer, and assist patients safely.
Ability to lift up to 50 pounds with or without reasonable accommodation.
Visual and auditory ability sufficient to perform patient assessments and documentation.
Ability to use electronic medical records and mobile documentation systems.

Work Environment

Patient homes throughout the agency's service area.
Exposure to varying home environments and weather conditions.
Frequent local travel required.
