Full Job Description

JOB/ PROGRAM DESCRIPTION:

Instituto Familiar de la Raza, Inc. (IFR) is the lead agency to Roadmap to Peace (RTP) initiative, a community-driven initiative to advance economic, health and safety outcomes for CLI identified youth ages 13-24 citywide. RTP vision is for community-informed solutions that are restorative, focused on trauma and healing, and offer timely, culturally responsive, and coordinated holistic services for those most impacted by violence. The RTP initiative is a collaboration of five community-based service providers and has three interrelated strategies: Healing through a culturally responsive service network, community building activities, and policy reform. We support every youth/young adult to get back on their path to healing and wholeness through a wrap-around care model while supporting a sense of belonging with community activities and events. The RTP Vision is that San Francisco’s Latinx youth most vulnerable to violence, heal, transform and thrive. This position works closely with IFR’s La Cultura Cura Program.

For decades, IFR has been part of a long-term violence prevention/peace building effort in the Mission District. RTP was born as a community-driven vision to address youth and young adults holistically. RTP leverages IFRs mental and behavioral health services and provides CLI youth care management and mental health services. Specifically, services provided include individual/family/group and community interventions; mental health consultation to violence prevention staff in other agencies; parent/caregiver engagement and psycho-educational interventions to build protective factors and reduce risk factors related to untreated trauma among youth and family; community level interventions including traditional (ancestral) approaches. Some examples include Drumming for Peace, Healing Circles, and cultural affirmation workshops.

The Care Manager is part of IFR’s La Cultura Cura Program AND works within the Roadmap to Peace Initiative. The care manager works with other RTP initiative care managers, service providers and core staff. The position offers a range of services and opportunities to youth/young adults and their families with a particular emphasis on individuals that have had contact with/or are at high risk of entering the juvenile justice system and/or impacted by violence. Youth and families are provided with case management and mental health services in the community.

IFR is seeking someone with experience delivering care management services to youth and their families utilizing restorative practices and trauma informed interventions. The ideal applicant will work with native born and immigrant CLI youth that are out-of-school and/or system-touched and in risk for negative street activity. The care manager will be part of a holistic network of services to advance the economic, health and safety outcomes for CLI youth/young adults served.

This position requires being co-located (RTP and La Cultura Cura offices) and is supervised by La Cultura Cura administrative staff. A successful candidate must be bi-lingual in both English and Spanish. They will embrace a youth development approach and act in accordance with IFR philosophy, values and best practices.

Essential Job Duties

Assessment, Care Planning & Care Management

· Conducts screening and comprehensive assessments of youth participating in Roadmap to Peace to identify challenges, strengths, needs, and readiness for RTP services.

· Develops and implements Individual Service Care Plans in partnership with youth and their families/caretakers.

· Coordinates care management activities to support identified goals and service needs.

· Provides care coordination, including Child and Family Team Meetings and/or Public Defender meetings as needed.

· Participates in care management team meetings to ensure coordinated service planning and a holistic approach to youth and family needs.

· Facilitates connections to other RTP services and/or community resources to address participant needs.

· Documents all encounters and contacts in participant records and maintains required statistical data.

Behavioral Health & Clinical Collaboration

· Works in conjunction with the Mental Health Clinician to address participants’ behavioral health needs.

· Actively participates in twice-monthly clinical consultation with the Mental Health Clinician to:

o Discuss client capacity, caseload dynamics, and complex cases.

o Navigate complex ethical and clinical decisions.

o Assess client progress toward goals and readiness for services.

o Strengthen effective and healthy intervention practices.

· Collaborates with the broader care team to support participants' behavioral health, wellness, and service needs.

Juvenile Justice, Court Advocacy & Multidisciplinary Collaboration

· Participates in juvenile justice and adult collaborative courts as part of a multidisciplinary team.

· Supports and advocates for youth/TAY during juvenile and adult court proceedings.

· Participates in Public Defender meetings and other justice-system-related care coordination activities as needed.

· Collaborates with justice-system partners, service providers, families, and other stakeholders to promote positive outcomes for participants.

Community Engagement, Outreach & Resource Connection

· Conducts designated outreach activities, including school, home, and community visits.

· Builds partnerships and collaborative relationships that contribute to positive economic, health, and safety outcomes for youth.

· Facilitates connections to community-based resources and services.

· Supports RTP participant community activities, including day, evening, and weekend trips when appropriate.

· Participates in violence prevention activities, community events, and other outreach activities as requested.

Youth Development, Cultural Affirmation & Engagement

· Develops and implements an eight-week cultural affirmation group for RTP participants annually.

· Engages youth and families in culturally responsive, strengths-based service planning and interventions.

· Supports participant engagement in RTP activities and services that promote personal development, community connection, and positive outcomes.

· Participates in RTP Academy sessions, annual town halls, and community-hosted activities and events.

Documentation, Data & Program Participation

· Completes monthly reports regarding client participation and service engagement.

· Maintains accurate and timely participant records and required program documentation.

· Maintains statistical and programmatic data as requested, including participation in data-entry support spaces such as Data Cafés.

· Participates in collaborative meetings, inter-agency trainings, and other programmatic meetings and events.

· Participates in mandatory monthly Service Network meetings, RTP Academy sessions, annual town halls, and community-hosted activities/events.

· Bilingual English-Spanish

· Minimum three years of experience providing services to youth interfacing with justice systems and/or social services

· Experience working with Probation system, the Public Defender Office, and the "re-entry" population

· Experience with restorative justice and trauma informed practices

· Able to manage an active care load as assigned

· Must be able to respond to issues of diversity in the Latinx/Chicanx/Indigena community

· Experience working in partnership with other community service agencies and/or organizations

· Good written and oral presentation

· Communication skills – able to interact easily with Youth/TAY as well as with adults

· Ability to focus, manage time wisely, work independently and complete tasks on time

· Must be a Community-minded individual

· Knowledgeable of San Francisco

· Knowledgeable of youth at risk/in-risk experience

· Committed to a healthy lifestyle

· Skills utilizing Microsoft Word and Google Workspace platforms
