Full Job Description

Education: Medical degree from an accredited medical school or equivalent experience. Current Puerto Rico’s medical license.Eight (8) to (10) ten years in the healthcare environment, population management, and/or case management programs. Current Puerto Rico driver’s license and vehicle available to drive for work between locations if necessary.

Documents and Credentials established. Ability to speak, read and write in English and Spanish, must be Fully Bilingual in English and Spanish. Ability to write reports in both English and Spanish. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

General Description

1. Performs In-home visits or telemedicine for CHRA, Transition of Care, Caps in Care or medical evaluation of patients as per program needs. Provides Care Plan Recommendations for the care management of the patient.

2. Documents all Initial Progress Notes and Follow up Progress note on Varmed EHR.

3. Documents CHRA on MCS Platform (Provinet) in no more than 10 days after the evaluation. Responds all the “Return to Physician” CHRA in Provinet.

4. Provides on the floor clinical support to nurses in the Call Center if required and support case discussions, care plan reviews, and population management efforts as well as training efforts.

5. Functions as medical advisor for effective care integration of the interdisciplinary team, Case/Disease Management activities, quality improvement activities, and provider relations functions.

6. Participates in Case Discussion with clinical staff of the client.

7. Liaison with Medical Providers to intervene in administrative situations (physicians not contacted, limited access to PCP’s, among others).

8. Provide support to Care Managers to resolve day to day clinical situations (ex. Medical order for patients, administrative processes of health insurance, quality measurements, among others).

9. It is expected that the Medical Doctor maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies.

10. Performs all other duties as requested within the contract scope.

Opertional Responsabilities and metrics

1. Comply with Implementation and Work plans deadlines and deliverables.

2. Comply with established policies, processes, and procedures for the area.

3. Comply with Workload Requirements for the area.

4. Comply with the telemedicine / visits required percentage of Care Plans assigned.

5. Conduct the required Case discussions.

Compensation/benefits will be discussed upon education & experience
