Department: Reentry Programs and Services

Salary range $22.00-$26.00

Hours: Mon-Fri (occasional evenings and weekends)

Employment type: FTE (8 -4 M-F)

Reporting to: Extended Care Manager I

Organization Overview

Restorative Partners, Inc. (RP) is a nonprofit dedicated to transforming lives affected by crime through healing services and supportive relationships. Through education, compassion, and restorative practices, we strive to break cycles of harm and build a more just community.

Position Overview

As part of the CalAIM team, the ECM Care Manager assists CenCal Health Members in coordinating comprehensive care as an integral part of an interdisciplinary team to address physical and mental health, substance use, social needs, and long-term services, support, and accompaniment to people returning to San Luis Obispo County from incarceration through the Enhanced Care Management. The ECM Care Manager requires outreach, engagement and ECM program enrollment of new clients, completing comprehensive assessments, developing care management plans, coordinating care and social support. Key responsibilities include, but are not limited to, an in-depth knowledge of the CalAIM Enhanced Care Management and Community Supports programs. This role requires strong communication skills, empathy, prior care coordination or intensive case management experience, and a commitment to Restorative Partners’ mission.

Roles & Responsibilities

Client Support and Case Management

Case Management (Required)
ECM Experience (Preferred)
Maintain an average caseload of participants as determined by management (approx 25 - 30 clients).
Conduct initial outreach and engagement to establish rapport, determine program eligibility, ensure participant consent to services, and secure CenCal ECM program enrollment and authorization
Conduct intake and individualized needs assessments to develop plans that address participant's needs in the areas of physical health, mental health, SUD, community-based Long-Term Services Support, oral health, social supports, Social Determinants of Health.
Participate in RP’s regular interdepartmental meetings to collaborate and coordinate with other team members regarding care plans, supports, and referrals for participants
Support participant’s engagement in coordination of scheduling appointments, appointment reminders, coordinating transportation, identify and address other barriers to participant’s engagement in services
Assist participants in addressing immediate needs such as clothing, transportation, and vital documents.
Coach members to make lifestyle choices based on healthy behavior
Support members in strengthening their skills to identify and access resources to assist them in managing and prevention of chronic condition
Identify supports needed for each participant and coordinate referrals to appropriate services and community resources.
Assist members in accessing additional benefits and related documentation such as, Social Security Insurance (SSI), CalFresh, cash aid, and obtaining required documentation to apply (ID, birth certificate, proof of medical conditions, etc.). Accompany participants to critical appointments as determined by management
Maintain regular communication with each participant’s care team, including medical, behavioral health, and social service providers, to support coordinated, client-centered care.
Maintain confidentiality of participant files per HIPAA and all applicable guidelines

Community Engagement and Education

Participant outreach and engagement, including direct communication with participants such as in person meetings, mail, email, texts and telephone; community and street-level outreach
Represent the organization at community events and meetings to raise awareness about our services

Administrative Duties

Complete and maintain required ECM documentation, including comprehensive assessments and care plans, and track enrollments, case notes, referrals, and services in Apricot and to ensure records are accurate, organized, up to date, and compliant with program, CenCal and HIPAA requirements.
Prepare and submit regular reports on program activities, client progress, and outcomes.
Support tracking and reporting for CalAIM invoicing.

Professional Development

Stay updated on best practices in social work and relevant fields through ongoing education and training
Attend required workshops, webinars, and conferences to enhance skills and knowledge
Participate in regular supervision

Essential Requirements

Current driver's license, valid auto insurance, and reliable transportation
Successful completion of a comprehensive background check, as verified by law enforcement administration or other designated agency
Bachelor's degree in Social Work, Psychology, Criminal Justice, or related field preferred
Proficiency in Google Suite, Microsoft Office Suite, and database management systems
Ability to work flexible hours, including occasional evenings and weekends
Fluency in English (bilingual English/Spanish highly desirable)

The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. This position requires the ability to occasionally lift up to 20 pounds. Work Environment

This job operates primarily in a professional office environment and routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.
