Job Summary
We are seeking a dynamic and dedicated Service Coordinator to join our team, focusing on the NHTD (Nursing Home Transition and Diversion) and TBI (Traumatic Brain Injury) Waiver Program for Nassau and Suffolk counties (Long Island). In this vital role, you will serve as a key point of contact for individuals with developmental disabilities, coordinating services that promote independence, community integration, and overall well-being. Your energetic approach will ensure seamless program delivery, foster strong relationships with clients and providers, and support the successful management of program operations. This paid position offers an exciting opportunity to make a meaningful difference in the lives of those we serve while developing your expertise in program development, management, and social work.

Responsibilities

Act as the primary liaison for individuals enrolled in the NHTD/TBI Waiver Program, ensuring their needs are accurately assessed and met through personalized service plans.
Coordinate with healthcare providers, families, and community agencies to develop comprehensive care strategies that promote independence and quality of life.
Oversee program operations by managing documentation, maintaining client records, and ensuring compliance with all regulatory requirements.
Develop and implement program initiatives aimed at enhancing service quality, efficiency, and client satisfaction.
Monitor budgets related to service provision, ensuring resources are allocated effectively while maintaining fiscal responsibility.
Conduct regular reviews of client progress and adjust care plans accordingly to meet evolving needs.
Manage relationships with stakeholders by providing exceptional customer service, resolving issues promptly, and building trust within the community.

Qualifications

Proven experience in management or program development within social services or healthcare settings.
Strong background working with individuals with developmental disabilities or behavioral health needs.
Demonstrated ability to build and maintain positive relationships with clients, families, providers, and community partners.
Excellent organizational skills with experience in administrative tasks such as documentation, record keeping, and budgeting.
Knowledge of social work principles, program management practices, and behavioral health concepts.
