Full Job Description

Young Lives Redeemed is an Orange County-based nonprofit dedicated to providing essential support and services to homeless former foster youth ages 18-25. Our mission is to empower program participants to achieve self-sufficiency, stability, and a brighter future. We are seeking a passionate, dedicated, and experienced Case Manager, who will work with our female participants, to join our team and make a meaningful impact on the lives of the transitional age youth we serve.

Position Overview:

As a Case Manager for our female participants, you will play a crucial role in helping former foster youth successfully transition into independent living. You will provide comprehensive case management services, guidance, and support to empower these young adults to overcome challenges and achieve their goals.

Key Responsibilities:

Assessment and Planning: Conduct comprehensive assessments of each participant's needs, strengths, and barriers to independence. Develop individualized service plans to address these needs.
Intensive Management: Invest deeply in each participant's journey—spending several hours each week providing consistent, hands-on, face to face time supporting their daily routines, challenges, and personal growth.
Advocacy: Serve as an advocate for clients in accessing housing, healthcare, education, employment, and other essential services. Help them navigate complex systems to build self-confidence and self-advocacy skills to become more self-reliant.
Crisis Intervention: Provide immediate support and intervention in crisis situations, such as homelessness, mental health issues, or substance abuse.
Life Skills Training: Deliver life skills workshops and one-on-one coaching sessions to help clients develop essential skills, including budgeting, time management, and goal setting.
Monitoring Progress: Continuously track and monitor participants progress towards their goals. Adjust service plans as needed to ensure successful outcomes.
Conflict Resolution and Client Accountability: Navigate difficult conversations with empathy and professionalism, setting clear boundaries while using de-escalation and conflict-resolution skills to maintain safety, trust, and accountability.
Resource Coordination: Collaborate with community partners, agencies, and organizations to connect participants with available resources and opportunities.
This position requires in-person attendance and is not a remote or hybrid role.

Qualifications:

Bachelor's degree in social work, psychology, counseling, or a related field preferred.
Experience working with individuals challenged by homelessness, mental illness, and or substance use.
Strong understanding of trauma-informed care and the unique challenges faced by this population.
Excellent communication and interpersonal skills.
Empathy, patience, and the ability to establish trust with clients.
Knowledge of local resources and services for homeless youth.
Ability to work independently and as part of a team.
Flexibility and adaptability in a fast-paced, dynamic environment.
Female applicants prefered.

Compensation:

$23.00-$25.00 per hour depending on experience.
