Location: Sunland

Shift: Mon-Fri 7:30am-2:30pm

Employee Type: Full Time Hourly Program Coordinator

Degree: BA in Human Services- required

Pay Rate: $23.43

We are seeking a compassionate, organized, and motivated Program Coordinator to support the daily operations of our Adult Day Program. In this role, you will work closely with the Program Director to coordinate staff, oversee daily program activities, support participant care, and ensure high-quality, person-centered services. Job Summary:

The Program Coordinator assists the Program Director in the daily operation of the Adult Day Program by coordinating participant services, staff assignments, scheduling, documentation, and compliance activities. This position works collaboratively with the interdisciplinary team, including nursing and day program staff, to ensure the delivery of high-quality, person-centered services that promote participants’ health, safety, independence, dignity, and quality of life. The Program Coordinator plays a key role in program operations, documentation, report writing, quality assurance, participant support, and staff coordination while helping ensure regulatory compliance and efficient program flow.

Job Duties:

· Assist the Program Director with daily program operations and coordination.

· Develop, create, and generate weekly and daily staff schedules to ensure adequate coverage for all program activities.

· Support the Program Director by creating daily staff assignments and task distribution.

· Supervise program aides and ensure compliance with break schedules, assignments, and coverage requirements.

· Assist program groups as needed to ensure adequate staffing and continuity of care.

· Provide direct floor coverage as needed to maintain staffing ratios and program continuity.

· Monitor daily operations to ensure quality services and adherence to agency policies.

· Assist with onboarding, training, and mentoring new staff on program procedures and documentation standards.

· Serve as a resource to direct care staff regarding participant care and program operations.

· Assist participants with activities of daily living (ADLs), including hygiene and personal care as needed.

· Facilitate therapeutic, recreational, and social activities that promote engagement and independence.

· Observe participants for changes in condition and report concerns to appropriate interdisciplinary team members.

· Support implementation of individualized care plans developed by the interdisciplinary team.

· Promote participant dignity, rights, independence, and person-centered care at all times.

· Assist with documentation & report writing

· Complete accurate, timely, and objective participant progress notes in accordance with program requirements.

· Assist with participant profiles, assessments, annual/semi reports, care plan documentation, and discharge summaries.

· Assist with maintaining participant records to ensure accuracy, completeness, and audit readiness.

· Assist with admissions, transfers, and discharges of participants.

· Participate in mandatory trainings and continuing education requirements.

· Perform other duties as assigned by the Program Director or Administrator.

ACCOUNTABILITIES

· Report suspected abuse, neglect, or exploitation immediately per mandated reporting requirements.

· Respond appropriately to emergencies following agency procedures.

· Maintain current CPR/AED certification annually and First Aid certification as required.

· Maintain required health screenings, TB testing, and immunizations per policy.

· Follow all infection control, safety, and HIPAA regulations.

· Maintain professional conduct, appearance, and ethical standards at all times.

Requirements:

· Bachelor’s degree in Human Services, Healthcare Administration, Psychology, Gerontology, or a related field required.

· High School Diploma or GED required.

· Minimum one (1) year of experience in an Adult Day Program, Adult Day Health Care, healthcare, or social services setting.

· Experience with documentation, report writing, or program coordination preferred.

· Strong written communication skills with ability to prepare professional reports and documentation.

· Strong organizational and time management skills with ability to meet deadlines.

· Proficiency in Microsoft Office (Word, Excel, Outlook) and electronic documentation systems.

· Ability to communicate effectively with participants, consumers, families, and interdisciplinary team members.

· Ability to maintain accurate documentation

· Strong interpersonal skills and ability to work both independently and as part of a team.

· Ability to read, write, and communicate effectively in English.

· Must pass required background checks and health screenings.

· CPR/AED and First Aid certification required and maintained.

· Ability to maintain professional appearance and conduct.
